Extracellular Vesicles: New Classification and Tumor Immunosuppression

Summary: Extracellular vesicles (EVs) are cell-derived membrane-surrounded vesicles that carry bioactive molecules and deliver them to recipient cells. They can be classified into classical EVs (exosomes, microvesicles, and apoptotic bodies) and additional types (autophagic EVs, matrix vesicles, and stressed EVs). EVs are involved in the communication between cancer cells and tumor-associated cells (TAC), leading to the establishment of immunosuppressive and chemoresistant microenvironments. Cancer EVs play important roles in immunosuppression, immune evasion, and cellular transformation, and can change hot tumors into cold ones.

Loading cargo into exosomes, however, remains challenging despite various strategies developed. This review provides an overview of current cargo delivery strategies using exosomes and summarizes recent approaches for loading small-molecule, nucleic acid, and protein drugs into exosomes, offering insights for more efficient drug delivery.

Additionally, exosomes can trigger chronic inflammation and lead to immune evasion and tumor progression by transferring RNA and modulating molecular signaling pathways. Moreover, exosomes can serve as carriers of anti-tumor agents and genetic tools to control cancer progression. Finally, exosomes are valuable diagnostic and prognostic tools in the treatment of cancer patients.
