A Real-World Analysis of High-Frequency 10 kHz Spinal Cord Stimulation for the Treatment of Painful Diabetic Peripheral Neuropathy

Jeffrey L. Chen et al.

Summary: This retrospective study assessed the pain relief and functional improvements in diabetic neuropathy patients who were implanted with a high-frequency spinal cord stimulation device. The results showed that 10 kHz SCS provided meaningful pain relief for a majority of patients and improved sleep and overall function.

Neuromodulation Interventions for the Treatment of Painful Diabetic Neuropathy: a Systematic Review

Ryan S. D'Souza et al.

Summary: Painful diabetic neuropathy is a prevalent and debilitating condition characterized by severe pain, poor quality of life, and various physical and psychological issues. Recent studies have found that 10-kHz and tonic spinal cord stimulation can help alleviate pain and improve patient satisfaction, while evidence for other neuromodulation interventions is limited.

The state-of-the-art pharmacotherapeutic options for the treatment of chronic non-cancer pain

Ryan S. D'Souza et al.

Summary: In the era of the opioid epidemic, clinicians are advised to prioritize non-opioid analgesics and employ a multimodal analgesic approach. Personalized treatment based on individual factors such as type, location, severity, and chronicity of pain is recommended, taking into consideration the risk-to-benefit ratio in each case. It is important for clinicians to carefully evaluate drug side effects, patient age, and comorbidities when determining the appropriate pharmacotherapeutic options for chronic non-cancer pain.

Evidence-Based Treatment of Painful Diabetic Neuropathy: a Systematic Review

Ryan S. D'Souza et al.

Summary: This article evaluates the evidence for conservative, pharmacological, and neuromodulation treatments for painful diabetic neuropathy (PDN). It is found that intensive glycemic control with insulin may reduce the risk of distal symmetric polyneuropathy in patients with type 1 diabetes. First-line pharmacological treatments for PDN include gabapentinoids and duloxetine.

Neuromodulation Therapy for Chemotherapy-Induced Peripheral Neuropathy: A Systematic Review

Ryan S. D'Souza et al.

Summary: This study conducted a systematic review to assess the impact of neuromodulation therapy on pain intensity and neurological function in patients with chemotherapy-induced peripheral neuropathy (CIPN). The results suggest that dorsal column spinal cord stimulation (SCS), dorsal root ganglion stimulation (DRG-S), and peripheral nerve stimulation (PNS) may reduce pain severity in CIPN patients, but the effects on neurological function remain uncertain.

Stimulation holiday rescues analgesia after habituation and loss of efficacy from 10-kilohertz dorsal column spinal cord stimulation

Ryan S. D'Souza et al.

Summary: This study investigated the effectiveness of a stimulation holiday strategy in rescuing analgesia from spinal cord stimulation failure. The results showed that restarting spinal cord stimulation after a stimulation holiday significantly improved pain relief and responder rate.

Rezvan Ahmadi et al.

Summary: The new wireless spinal cord stimulation (SCS) technology introduced promises minimal invasive SCS with a wide range of stimulation paradigms and MRI conditionality. Evaluation of 12 patients with therapy-resistant neuropathic pain showed that wireless SCS systems were feasible and safe, with most patients achieving 50% or more pain relief.

Mechanisms for the Clinical Utility of Low-Frequency Stimulation in Neuromodulation of the Dorsal Root Ganglion

Kenneth B. Chapman et al.

Summary: This review explores the effectiveness of low-frequency DRG-S for pain relief, finding that low-frequency stimulation activates inhibitory processes while high-frequency stimulation is excitatory. The authors propose that very low-frequency DRG-S partially utilizes LTMRs and the endogenous opioid system.

Summary: This study analyzed randomized trials of analgesic interventions after total hip or knee arthroplasty, identifying MCIDs for pain scores and rescue opioid consumption. However, a significant proportion of trials still had inadequate sample size calculations, and a portion of significant results did not reach the predetermined MCID.
