The anomalous pharmacology of fentanyl

Eamonn Kelly et al.

Summary: Fentanyl is a key therapeutic drug used in anaesthesia and pain management, but its illicit use has led to a significant increase in opioid overdose deaths. This review examines the unique pharmacological properties of fentanyl and explores whether these properties contribute to overdose deaths involving fentanyl.

Continuous peripheral nerve blocks for analgesia following painful ambulatory surgery: a review with focus on recent developments in infusion technology

John J. Finneran et al.

Summary: This review focuses on the history and evolution of ambulatory continuous peripheral nerve blocks (cPNBs), recent developments in infusion technology to prolong the duration of analgesia, optimal choice of cPNB for different surgical procedures, and novel analgesic modalities as alternatives or supplements to cPNBs. Recent evidence suggests that using automated boluses instead of continuous infusions may decrease the consumption rate of local anesthetics and prolong the duration of analgesia. Utilizing long-acting local anesthetics and start-delay timers for infusion may further increase the duration of the benefits.

A Systematic Review of Randomized Clinical Trials Evaluating the Efficacy of Minimally Invasive Surgery for Soft Tissue Management: Aesthetics, Postoperative Morbidity, and Clinical Results

Carlos M. Ardila et al.

Summary: This systematic review evaluated the efficacy of minimally invasive surgical technique (MIST) for soft tissue management. The results showed that MIST had better outcomes in preserving papilla height, managing excessive gingival display, and single implant placement. It also demonstrated improved root coverage and high patient satisfaction. The MIST group had significantly less post-surgical pain and better wound healing scores.

Clinical and Microbiological Efficacy of Adjunctive Systemic Quinolones to Mechanical Therapy in Periodontitis: A Systematic Review of the Literature

Carlos-M. Ardila et al.

Summary: This study evaluates the clinical and microbiological efficacy of systemic quinolones adjunctive to mechanical therapy in periodontitis patients. The results show that adjunctive use of moxifloxacin and levofloxacin improves probing depth and clinical attachment level compared to subgingival debridement alone, and they also have better efficacy against A. actinomycetemcomitans.

Transdermal fentanyl patch versus standard analgesia in postoperative oral submucous fibrosis patients: a triple blinded, randomised control trial

P. M. Nihadha et al.

Summary: Severe pain in oral submucous fibrosis (OSMF) patients hinders their physiotherapy and negatively impacts surgical outcomes and patient compliance. This study aimed to develop a postoperative pain control protocol for managing OSMF. The randomized controlled trial compared the effects of non-opioid analgesics (NSAIDs) and transdermal fentanyl patches (TFP) on pain and interincisal opening in OSMF patients. The findings revealed that TFP had a significant effect in controlling severe pain during active mouth opening exercises and improved patient compliance. However, there were no significant differences in mouth opening and quality of life between the two groups. Therefore, the study recommends the use of TFP for better pain control and compliance in postoperative OSMF patients.

Is Europe Facing an Emerging Opioid Crisis Comparable to the US?

Jan van Amsterdam et al.

Summary: The United States is facing an opioid overdose crisis due to over-prescription and aggressive marketing, while in Europe, although prescription opioid use has increased, there is no evidence of a similar crisis except in Scotland. Authorities should be vigilant despite declining prescription rates in Europe.

Illicit synthetic opioid consumption in Asia and the Pacific: Assessing the risks of a potential outbreak

Jirka Taylor et al.

Summary: The research found that Australia has three facilitating factors for the encroachment of IMPSO into the market, while the other countries mainly have existing illicit opioid markets. Although diverted pharmaceutical opioids are a problem in Australia and India, heroin is the dominant opioid in all four countries. There are varying trends in heroin use, with a decrease in China, an increase in India, and stability in Australia and Myanmar.
