Topographical differences in white matter hyperintensity burden and cognition in aging, MCI, and AD

Summary: This study examined the regional differences of white matter hyperintensities (WMHs) between normal controls (NCs), people with mild cognitive impairment (MCI), and Alzheimer's disease (AD), and also investigated the association between WMHs and cognitive decline. The results showed that MCI and AD patients had increased total and regional WMHs compared to NCs, and there was an association between WMHs and cognition in NCs in all regions. MCI patients showed a steeper decline in all cognitive domains compared to NCs, while AD patients only showed a steeper decline in global cognition in the temporal region.

Summary: The study found that cerebral small vessel disease (CSVD), amyloid, and pTau are related to age-related cognitive decline. Specifically, there was an inverse correlation between white matter lesions (WMLs) and amyloid AB42, but no correlation between WMLs and pTau. Additionally, baseline AB42 was associated with executive function, while baseline pTau did not have any association with cognitive measures.

Axonal loss in the frontal lobe was associated with hyperphosphorylated tau, and calpain levels were associated with increasing amyloid-beta burden in the AD group, indicating an additional degenerative influence.
