Fertility, pregnancy and childbirth in women with multiple sclerosis: a population-based study from 2018 to 2020

Marcello Moccia et al.

Summary: This study evaluated whether fertility, pregnancy, delivery, and breastfeeding have actually improved in women with multiple sclerosis (MS), compared with the general population, and in relation to treatment features. The results showed that the fertility rate in women with MS remains lower than in the general population. Continuing disease-modifying treatment (DMT) during pregnancy was associated with lower birth weight, while exposure to DMTs with unknown/negative effects on pregnancy was associated with birth defects. Additionally, women treated with breastfeeding-safe DMTs had a higher probability of breastfeeding.

Pregnancy-induced brain magnetic resonance imaging changes in women with multiple sclerosis

Tomas Uher et al.

Summary: The study aimed to investigate the brain changes and radiological disease activity in women with multiple sclerosis (MS) during pregnancy and postpartum periods. The results showed that women in the postpartum period had increased T2 lesion volume and accelerated brain volume loss, which is important for treatment decision-making and clinical trial design.

Estimating Risk of Multiple Sclerosis Disease Reactivation in Pregnancy and Postpartum: The VIPRiMS Score

Gabriel Bsteh et al.

Summary: The study aims to generate and validate a risk score for disease reactivation in patients with relapsing multiple sclerosis (RMS) during pregnancy. By analyzing predictors including relapse occurrence, DMT type, DMT washout duration, EDSS at conception, and time until DMT restart postpartum, a predictive score was developed to identify patients at high, intermediate, and low risk of disease reactivation during pregnancy and up to 6 months postpartum.

Effects of Pregnancy and Breastfeeding on Clinical Outcomes and MRI Measurements of Women with Multiple Sclerosis: An Exploratory Real-World Cohort Study

Lorena Lorefice et al.

Summary: This study explored the effects of pregnancy and breastfeeding on short- and long-term outcomes for women with multiple sclerosis (MS). The results showed that post-partum clinical activity was associated with clinical activity before conception and duration of breastfeeding. Having had at least one pregnancy during MS was linked to a lower EDSS score, while breastfeeding duration longer than 6 months was associated with lower white matter volume.

Brain MRI activity during the year before pregnancy can predict post-partum clinical relapses

Hillel Lehmann et al.

Summary: The study confirmed a significant correlation between active MRI pre-pregnancy and post-partum relapse, as well as a correlation between pre-pregnancy EDSS and post-partum relapse. Using a multivariate model, it was predicted that women with certain pre-pregnancy characteristics will not experience post-partum relapses.

Clinical and Radiologic Disease Activity in Pregnancy and Postpartum in MS

Annika Anderson et al.

Summary: In women with multiple sclerosis, the relapse rate decreases during pregnancy but increases in the 3 months postpartum. Postpartum clinical relapses are associated with inflammation on MRI, and even without clinical relapses, new lesions can be detected on brain MRIs.
