The value of convolutional neural networks-based deep learning model in differential diagnosis of space-occupying brain diseases

Xiuling Miao et al.

Summary: This study aimed to evaluate the value of a CNN-based deep learning model in the differential diagnosis of space-occupying brain diseases on MRI. The results showed that the model had higher accuracy than senior neuroradiologists and could accurately identify radiographic features of tumefactive demyelinating lesions, tumefactive primary angiitis of the central nervous system, primary central nervous system lymphoma, and brain gliomas.

Clinical, laboratory, and radiological features of cerebral amyloid angiopathy-related inflammation (CAA-ri): retrospective, observational experience of a single centre

Dan Li et al.

Summary: In this observational study, 15 patients with CAA-ri were analyzed retrospectively. The study revealed that cognitive decline, focal neurologic deficit, and headache were the most common clinical manifestations. The imaging results showed that WMH lesions were rarely seen in the cerebellum and brainstem, and no hemorrhagic lesion was observed in the brainstem. After immunotherapy, most patients showed improvement or stability in clinical and radiological outcomes.

Eosinophilic granulomatosis with polyangeitis

Carlos Romero Gomez et al.

Summary: EGPA is a systemic vasculitis characterized by asthma, eosinophilia, eosinophilic infiltration, and vasculitis. It is less frequently associated with anti-neutrophil cytoplasmic antibody (ANCA). The disease is rare, occurring in patients with asthma and affecting multiple organs. Treatment involves the use of glucocorticoids and immunosuppressants. Recent progress has been made in understanding the pathophysiology, incorporating biologic agents in treatment, revising classification criteria, and publishing new therapeutic recommendations.

Summary: High-resolution magnetic resonance imaging (HRMRI) is the most important and popular technique for evaluating vessel wall and cerebral arterial disease. It can identify stroke causes, differentiate various diagnoses, and provide additional disease information through noninvasive visualization and assessment of the vessel wall. This report discusses technical considerations and reviews current clinical applications of HRMRI.

Central Nervous System Vasculitis Primary Angiitis of the Central Nervous System and Central Nervous System Manifestations of Systemic Vasculitis

Moein Amin et al.

Summary: CNSV is a group of disorders that affect the vasculature within the brain, spinal cord, and leptomeninges. It can present as either the primary disorder (PACNS) or secondary to another disorder (secondary CNSV). The diagnosis relies on a combination of history, physical examination, serological and CSF evaluation, MRI, vessel imaging, and sometimes histopathological evaluation. Treatment options include immunosuppression for PACNS or treatment of the underlying condition for secondary CNSV.

Focal status epilepticus may trigger relapse of primary angiitis of the CNS

Antoine Guilmot et al.

Summary: The short-term effects of seizures on established CNS pathologies are less studied and less predictable. This case study describes a woman with recurrent cerebral hemorrhage who developed a pseudo-tumoral edema after a seizure episode. The brain biopsy revealed primary angiitis of the CNS. Seizure-induced blood-brain barrier dysfunction can lead to the entry of water and inflammatory molecules, triggering a self-reinforcing process. Caution should be taken when tapering antiepileptic drugs in patients with such conditions.

Current clinical and therapeutic approach to tumour-like mass lesions in granulomatosis with polyangiitis

Roberto Padoan et al.

Summary: Granulomatosis with polyangiitis (GPA) is a systemic autoimmune disorder characterized by respiratory tract disease, vasculitis, and renal manifestations. Mass lesions, also known as granulomas, are rare manifestations of GPA. They may represent a localized or systemic manifestation and can lead to delayed diagnosis and organ damage. The management of GPA with mass lesions can be challenging, and alternative diagnoses such as malignancy or infection should be considered.

Recurrent ulcerative necrotising stomatitis in two dogs with concurrent steroid-responsive chronic rhinitis and suspected underlying oral vasculitis

Perrine Henry et al.

Summary: A 2-year-old female springer spaniel and an 8-year-old male Scottish terrier were referred for recurrent oral necrotic ulcers, pseudomembrane, and nasal discharge. Biopsy examinations revealed chronic ulcerative necrotizing stomatitis and chronic neutrophilic, eosinophilic, or plasmacytic rhinitis. Treatment with immunosuppressive agents provided temporary remission, but eventually, both dogs were euthanized due to the painful and recurrent nature of the condition.

Primary Angiitis of the Central Nervous System - Diagnosis and Management

Soumya Sundaram et al.

Summary: Primary angiitis of central nervous system (PACNS) is a rare disorder with non-specific clinical manifestations. Histopathological evidence is needed for accurate diagnosis. Understanding the patient's medical history, conducting physical examinations, neuroimaging, immunological testing, and cerebrospinal fluid analysis can help differentiate between other possible causes. Timely treatment with glucocorticoids and immunosuppressive agents is crucial for management.
