Intrathecal Onasemnogene Abeparvovec for Sitting, Nonambulatory Patients with Spinal Muscular Atrophy: Phase I Ascending-Dose Study (STRONG)

Summary: This study assessed the safety and efficacy of intrathecal onasemnogene abeparvovec in SMA patients. The results showed that intrathecal administration was safe and effective at an appropriate dose. Older patients showed significant improvement in HFMSE score after treatment.

Summary: The study showed early sensory neuron toxicity in Macaca fascicularis following IV delivery of AAV vectors at high doses, with rats showing toxicity after one month, suggesting inclusion of the autonomic system in preclinical and clinical studies. Rats may be useful for evaluating the potential DRG toxicity of AAV vectors, while acute hepatic toxicity associated with coagulation disorders appears to be highly species-dependent.

Intrathecal sc-AAV9-CB-GFP: Systemic Distribution Predominates Following Single-Dose Administration in Cynomolgus Macaques

Summary: In this study, the biodistribution of self-complementary adeno-associated virus-9 (scAAV9) was assessed in juvenile cynomolgus macaques. The results showed widespread distribution of the virus in the central nervous system and various systemic tissues, with the highest levels of vector DNA and green fluorescent protein (GFP) expression observed in the spinal cord, dorsal root ganglia, and liver. Transduction and expression were primarily observed in perivascular astrocytes in the brain, with low levels in neurons. Higher levels of GFP expression were observed in hepatocytes, striated myocytes, cardiomyocytes, spinal cord lower motor neurons, and dorsal root ganglia sensory neurons.

Analysis of data from 325 patients receiving OA treatment showed elevated liver function test results in some patients. Summary: Adeno-associated virus (AAV)-mediated gene transfer has shown benefits in treating patients with inherited diseases like hemophilia B, but challenges remain due to potential rejection of AAV-transduced cells. Immunosuppression may prevent rejection in some patients. CD8(+) T cells induced by AAV infections may recognize AAV vector's capsids and eliminate cells expressing degraded capsid antigens, or AAV vectors themselves may induce de novo T cell responses, particularly at high doses. This chapter discusses strategies to prevent activation of CD8(+) T cell responses to AAV infections and gene transfer.

Gene Therapy in Hemophilia: Recent Advances

Summary: Hemophilia is a monogenic mutational disease affecting coagulation factor genes, with recombinant clotting factors being the current preferred treatment. Advanced gene therapies show promise for a curative effect, but are associated with immunogenicity and hepatotoxicity that need to be optimized. It is important for both physicians and patients to understand the potential benefits and risks of new therapies, and to maintain a registry of gene therapy patients to track efficacy and long-term adverse events.
