Colistin Resistance in Acinetobacter baumannii: Molecular Mechanisms and Epidemiology

Summary: A. baumannii is a clinically significant pathogen causing various nosocomial infections. Colistin, as a last-resort antibiotic, is used to treat multidrug-resistant A. baumannii infections. However, several mechanisms of colistin resistance in A. baumannii have been reported, including loss of LPS, modifications of target LPS, and efflux of colistin. In addition, widespread heteroresistance in A. baumannii contributes to colistin treatment failure. This review critically assesses the molecular mechanisms and global distribution of colistin-resistant strains in A. baumannii based on more than 50 experimental papers.

In this study, we found that a combination of a frog-skin antimicrobial peptide, Esc(1-21), and colistin exhibited synergistic effects against colistin-resistant Acinetobacter baumannii clinical isolates, suggesting a potential clinical application of this combination therapy.

Summary: Acinetobacter baumannii, a Gram-negative opportunistic nosocomial pathogen, poses a serious threat to public health due to its resistance to common antimicrobials and tolerance to desiccation. Bacteria regulate their virulence behavior through a phenomenon called quorum sensing, and the use of quorum sensing inhibitors or quorum quenching enzymes has been developed to reduce bacterial virulence and drug resistance pressure. These inhibitors or enzymes have the potential to inhibit bacterial quorum sensing and biofilm formation.
