Idiopathic Pulmonary Fibrosis (an Update) and Progressive Pulmonary Fibrosis in Adults An Official ATS/ERS/JRS/ALAT Clinical Practice Guideline

Ganesh Raghu et al.

Summary: This article provides updates on the guidelines for idiopathic pulmonary fibrosis (IPF) and addresses the progression of pulmonary fibrosis in patients with interstitial lung diseases (ILDs) other than IPF. The recommendations are based on consensus and systematic reviews, aiming to provide evidence-based guidance for clinicians.

The Incidence and Prevalence of Pulmonary Hypertension in the COPD Population: A Systematic Review and Meta-Analysis

Limin Zhang et al.

Summary: Chronic obstructive pulmonary disease (COPD)-related pulmonary hypertension (PH) is a common comorbidity of COPD, and its prevalence varies worldwide. This study found that the overall prevalence of COPD-related PH was 39.2%, with a lower prevalence of severe PH. There were significant regional differences, with the highest prevalence in Africa and the lowest in Europe. Other factors such as age, sex, enrolment time, participant recruitment settings, and PH diagnostic methods did not show significant differences in prevalence.

Inhaled Treprostinil in Pulmonary Hypertension Due to Interstitial Lung Disease

Aaron Waxman et al.

Summary: In patients with pulmonary hypertension due to interstitial lung disease, inhaled treprostinil significantly improved exercise capacity and clinical outcomes compared to placebo in a 16-week trial. Adverse events such as cough, headache, dyspnea, dizziness, nausea, fatigue, and diarrhea were commonly reported.

Summary: Using mendelian randomisation, this study found that shorter telomere length is associated with higher odds of IPF but not COPD, suggesting a potential contributory factor in IPF and differing mechanisms in COPD.

Combined Pulmonary Fibrosis and Emphysema (CPFE) Clinical Features and Management

Rene Hage et al.

Summary: CPFE is an underrecognized syndrome characterized by chronic, progressive disease with a dismal prognosis. Pulmonary hypertension (WHO group 3) and lung cancer are frequent co-morbidities. Male sex and smoking are important risk factors. Further research is needed on CPFE, especially relating to etiology, genetic influence, treatment, and prevention options. Antifibrotic therapy might be an interesting treatment option for these patients.

Dysregulation of endocytic machinery and ACE2 in small airways of smokers and COPD patients can augment their susceptibility to SARS-CoV-2 (COVID-19) infections

Mathew Suji Eapen et al.

Summary: The study identified the presence of SARS-CoV-2 host attachment receptor ACE2 and endocytic vacuoles in the airways of smokers and COPD patients, indicating that these patient groups could be more susceptible to COVID-19. The findings suggest that the active presence of ACE2 and endocytic vacuoles in the small airways of smokers and COPD patients may increase their vulnerability to SARS-CoV-2 infection.
