Summary: Hepatitis D virus (HDV) coinfection with hepatitis B increases the risk of liver cancer, cirrhosis, and death compared to hepatitis B monoinfection. Accurate estimates of HDV prevalence are needed to effectively identify coinfected individuals. Implementing double reflex testing, including anti-HDV testing and HDV RNA testing, is the most effective method to generate estimates and identify undiagnosed individuals. This strategy is feasible and requires a relatively low number of tests.

Elimination of HDV: Epidemiologic implications and public health perspectives

Summary: Hepatitis delta virus (HDV) infection is the most severe form of viral hepatitis in humans. It is now recognized as a global public health infection, but accurate prevalence remains uncertain due to lack of good epidemiological studies. Under-screening for HDV is the main reason for underestimating the true burden of infection. Urgent efforts are needed to study HDV using reliable screening tools and surveillance methodologies.

Highly dynamic changes of regional HBV epidemiology over two decades

Summary: This study examined the epidemiology of HBV in a large single center cohort over the past two decades, revealing changes in the characteristics and trends of HBV infection. Differences were observed between Italian and non-Italian immigrants, with age and gender playing important roles in the infected population. The prevalence of chronic HBV decreased, while HBeAg-negative infection increased. The incidence of cirrhosis declined, except for patients with ongoing HDV infection, particularly among non-Italian immigrants.

Acute Delta Hepatitis in Italy spanning three decades (1991-2019): Evidence for the effectiveness of the hepatitis B vaccination campaign

Summary: The incidence of acute HDV in Italy declined significantly after the introduction of mandatory hepatitis B vaccination in 1991. Factors such as increasing age, decreasing male/female ratio, rising proportion of coinfections, declining rate of HBsAg positive acute hepatitis tested for by IgM anti-HDV, and an increasing number of cases among foreigners were observed. Changes in risky behaviors, such as intravenous drug use no longer being associated with HDV, were also noted. Considering the grey area concerning IgM-anti HDV testing, continued monitoring of acute HDV in foreigners is necessary.

The hepatitis D virus in Italy. A vanishing infection, not yet a vanished disease

Summary: Although HDV infection is disappearing in Italy, it still has a medical impact on liver transplantation due to the presence of residual cases among native Italian patients. The study found that a significant proportion of native Italians with HDV infection had cirrhosis, and a majority remained HDV viremic even after follow-up. Hepatitis D played a disproportionate role in liver transplants compared to its minor prevalence in HBsAg-positive liver disorders in Italy.
