Increased Susceptibility for Adverse Reactions to Ultrasound Enhancing Agents in Sickle Cell Disease

Melinda Wu et al.

Summary: This study characterized the pain-related adverse events (AEs) caused by ultrasound enhancing agents (UEAs) in patients with sickle cell disease (SCD) and investigated the potential mechanisms. The results showed that the pain caused by UEAs is mainly due to the reaction between UEAs and the patient's immune system, and is not directly related to vaso-occlusive crisis (VOC) caused by UEAs.

Determinants of severity in sickle cell disease

David C. Rees et al.

Summary: Sickle cell disease is a highly variable condition, with outcomes ranging from childhood death to relatively symptom-free living. The co-inheritance of alpha thalassemia and factors determining HbF levels significantly influence the phenotype, but few other significant genetic variants have been identified. Environmental factors, including socioeconomic status and access to medical care, play a significant role in determining outcomes. Exposure to cold and windy weather seems to trigger acute complications in many individuals, although the effects are unpredictable and vary with geography. Several studies have attempted to identify prognostic factors for predicting outcomes, particularly in infancy. Low hemoglobin, low hemoglobin F percentage, and high reticulocytes in childhood are associated with worse outcomes, but these effects are relatively weak and inconsistent.

A systematic literature review of frequency of vaso-occlusive crises in sickle cell disease

Ahmar U. Zaidi et al.

Summary: This study is the first to systematically assess the published evidence regarding VOCs in real-world settings. The reported frequency of VOCs varied widely, with many studies potentially underestimating the frequency. Consistent reporting of self-reported VOCs managed at home, definitions of VOCs, complicated VOCs, and duration of VOC episodes is needed.

Contrast-enhanced ultrasound: a comprehensive review of safety in children

Aikaterini Ntoulia et al.

Summary: Research has shown that adverse reactions in pediatric CEUS use are mainly non-serious, with a very small number of children experiencing allergic reactions. In studies involving intravenous CEUS, the rate of serious adverse events is low, while the rate of non-serious adverse events is slightly higher. Adverse events caused by the use of UCA in the bladder are mostly related to bladder catheters rather than the UCA itself.

Sickle cell vaso-occlusion: The dialectic between red cells and white cells

Nicola Conran et al.

Summary: Sickle cell anemia is a hereditary hemoglobinopathy characterized by abnormal hemoglobin polymerization that triggers pathological mechanisms leading to various severe complications. Recent research has focused on the role of leukocytes in vaso-occlusive processes and the importance of adhesion by different cell types, including endothelial cells, platelets, leukocytes, and red blood cells.

Definity, an affinity for painful crisis: a case series describing vaso-occlusive pain crises in sickle cell patients undergoing echocardiogram with Definity contrast

Alex D'Amico et al.

Summary: This case series highlights the potential risks of using ECAs in SCD patients, as both patients experienced vaso-occlusive crises after receiving the contrast agent. Further studies are needed to evaluate the safety profile of ECAs in this patient population.
