Trial of an Intervention to Improve Acute Heart Failure Outcomes

D. S. Lee et al.

Summary: The use of a hospital-based strategy to support clinical decision making and rapid follow-up for patients with acute heart failure led to a lower risk of death and hospitalization for cardiovascular causes within 30 days compared to usual care.

Health System-Level Performance in Prescribing Guideline-Directed Medical Therapy for Patients With Heart Failure With Reduced Ejection Fraction: Results From the CONNECT-HF Trial

Bradi B. Granger et al.

Summary: This study identified substantial gaps in the use of guideline-directed medical therapy (GDMT) even among high-performing healthcare systems. Hospital-level interventions were found to be ineffective in improving quality metrics, highlighting the need for a team-based approach to care and improved patient access to medications for successful postdischarge outcomes.

Temporal trends in mortality and readmission after acute heart failure: a systematic review and meta-regression in the past four decades

Antoine Kimmoun et al.

Summary: This study analyzed AHF studies with more than 100 patients from 1980 to 2017 and found a declining trend in 30-day all-cause death rates, although mortality and readmission rates remained high. The increase in the use of oral neurohormonal antagonists may be associated with improved survival after AHF episodes.

Summary: The study found that heart failure patients admitted directly or with a delay to critical care units had higher 30-day mortality and readmission rates, with patients experiencing delayed critical care admission having the highest risk of death and healthcare costs.

Summary: This study aimed to explore the influence of region and race/ethnicity on the effects of Empagliflozin in patients with heart failure. There were significant differences in placebo event rates for major HF events across diverse regions and race/ethnic groups, with the greatest benefit of Empagliflozin observed in groups with the highest ratio of HF hospitalization to cardiovascular death. Regional differences were attenuated when outpatient worsening HF events were included in the definition of HF events.

Misdiagnosis of Heart Failure: A Systematic Review of the Literature

Chun Wai Wong et al.

Summary: Heart failure (HF) misdiagnosis is common, often mistaken for chronic obstructive pulmonary disease (COPD). Conditions such as anemia and chronic kidney disease may increase the likelihood of unrecognized left ventricular systolic dysfunction. In addition to COPD, obesity, old age, atrial fibrillation, and ischemic heart disease are also common factors in the misdiagnosis of HF.

Summary: The majority of patients hospitalized for acute heart failure in Japan received intravenous diuretics, with some also receiving vasodilators or intensified therapies. The average length of stay was 23.3 days, with an in-hospital mortality rate of 13.2%. Patients requiring intensified therapies had the longest hospital stay and highest in-hospital mortality.
