The sympathetic nervous system exacerbates carotid body sensitivity in hypertension

Summary: This study investigated the role of sympathetic innervation in the hyperexcitability of carotid bodies (CBs) in spontaneously hypertensive (SH) rats. The results showed that the sympathetic innervation enhanced the CB-evoked sympathoexcitation in SH rats but not in Wistar rats. This enhanced sympathoexcitation appears to be mediated by α1-adrenoceptors located on blood vessels and/or glomus cells. These findings are important for understanding and controlling the pathophysiology of hypertension.

The findings provide a potential therapeutic angle for heart failure and its complications.

GLP1R Attenuates Sympathetic Response to High Glucose via Carotid Body Inhibition

Summary: The study found that GLP1R is expressed in the carotid bodies of rats and humans, and its decreased expression is associated with sympathetic hyperactivity. Targeted administration of GLP1R agonist can reduce the baseline discharge rate of the carotid body and attenuate blood pressure and sympathetic responses induced by chemoreflex. Importantly, GLP1R activation can eliminate hyperglycemia-induced peripheral chemoreflex sensitization and associated sympathetic overactivity.

Blood Pressure Regulation by the Carotid Sinus Nerve: Clinical Implications for Carotid Body Neuromodulation

Summary: The chronic electrical modulation of the carotid sinus nerve (CSN) through high-frequency alternating current can improve metabolic control in rat models of type 2 diabetes without significant impact on blood pressure. Activation of the chemoreceptors by hypoxia increases sympathetic activity, but compensatory mechanisms maintain blood flow and hemodynamic homeostasis. Surgical denervation or electrical block of the CSN eliminates the increase in sympathetic activity during hypoxia, but does not significantly affect blood pressure.

Summary: This study reveals that spinal thoracic sympathetic preganglionic neurons are activated by hypoxia and inhibited by hyperoxia, enhancing sympatho-respiratory activity induced by CNS asphyxia-like stimuli. The mechanism involving neuronal nitric oxide synthase 1 (NOS1) and nicotinamide adenine dinucleotide phosphate (NADPH) oxidase (NOX) is proposed. These findings suggest the importance of these spinal oxygen sensors (SOS) in regulating physiological responses to chronic disease, spinal cord injury, and cardiorespiratory crisis.

The presence of masked and white-coat hypertension in OSA patients emphasizes the importance of correct hypertension diagnosis.
