HIV Promotes Neurocognitive Impairment by Damaging the Hippocampal Microvessels

Summary: Current evidence suggests that mild cerebrovascular changes could induce neurodegeneration and contribute to HIV-associated neurocognitive disease (HAND) in HIV patients. This study investigated the impact of HIV infection on brain microvessels, focusing on hippocampal microvessels that are crucial for memory and cognitive abilities. The results showed that HIV infection led to oxidative stress, mitochondrial damage, and changes in microvessels in both in vitro and in vivo models.

The potential role of HIV-1 latency in promoting neuroinflammation and HIV-1-associated neurocognitive disorder

Summary: Despite the suppression of HIV-1 viral replication in the central nervous system by antiretroviral therapy, a significant percentage of HIV-1-infected patients still experience neuroinflammation and symptoms of HIV-1-associated neurocognitive disorder. Recent studies suggest that HIV-1 latency in microglia plays a role in both neuroinflammation and the progression of neurocognitive disorders. Therefore, anti-inflammatory drugs that can also inhibit HIV-1 reactivation may offer potential therapeutic options for treating these disorders.

HIV viral transcription and immune perturbations in the CNS of people with HIV despite ART

Summary: People with HIV on antiretroviral therapy experience neurological impairment, possibly due to viral or neuroimmune causes. Single-cell analysis of paired cerebrospinal fluid and peripheral blood revealed the production of HIV-1 RNA in central memory CD4+ T cells in the cerebrospinal fluid, as well as a higher frequency of infected cells in the cerebrospinal fluid compared to the blood. Additionally, rare microglia-like cells in the cerebrospinal fluid were identified using a reliable cell surface marker, CD204. Abnormal CD8+ T cell activation was found to be prevalent in the cerebrospinal fluid of people with HIV, suggesting ongoing CNS viral persistence and compartmentalized neuroimmune effects.

Summary: Damage to the cerebral vascular endothelium is a critical initiating event in the development of HIV-1-associated neurocognitive disorders. Exosomes isolated from HIV-infected cells accelerate dysfunction in brain microvascular endothelial cells by inducing mitochondrial hyperfusion and reducing nitric oxide synthase expression. These exosomes, containing viral proteins, reduce mitochondrial function and accelerate brain endothelial dysfunction.

Summary: The use of cART in HIV-infected individuals has led to increased life expectancy but also a higher prevalence of age-related comorbidities such as HIV-associated neurocognitive disorders. This study revealed that HIV TAT protein can induce a senescence-like phenotype in microglial cells, involving oxidative stress and inflammation. Modulation of the miR-505-SIRT3 axis could potentially reduce microglial senescence caused by HIV TAT.
