Machine Learning Methods Improve Specificity in Newborn Screening for Isovaleric Aciduria

Summary: Isovaleric aciduria (IVA) is a rare disorder of leucine metabolism that is included in newborn screening programs worldwide. However, the effectiveness of IVA screening is hindered by the identification of individuals with a milder variant of the disease and an increasing number of false positive results. In this study, machine learning methods were used to improve the classification of IVA, resulting in a significant reduction in false positive rates while maintaining high sensitivity.

Summary: This study evaluated the long-term clinical outcomes of 94 individuals with confirmed IVA, showing that NBS reduces mortality in classic IVA but does not reliably protect against severe neonatal metabolic decompensations. Individuals with mild IVA had excellent neurocognitive outcomes regardless of metabolic maintenance therapy, calling into question the benefit of NBS for this group. Uniform stratified therapeutic concepts are urgently needed.

The study presents a testing platform approach to confirm the functional implication of variants of uncertain significance (VUS) identified in newborns with suspected IVA.
