Linkage of national health and social care data: a cross-sectional study of multimorbidity and social care use in people aged over 65 years in Scotland

Summary: The study found that 93.3% of those receiving social care had multimorbidity, and only 16.2% of those with multimorbidity received social care. Age and multimorbidity had the strongest impact on receiving social care, with the oldest and most severe multimorbidity group having a 50% and 18% increased probability respectively compared to the youngest and those with no multimorbidity. Additionally, those in the most deprived areas had a 5.5% increased probability of receiving social care compared to the least deprived.

Assessment and management of multimorbidity should start in midlife and possibly earlier in disadvantaged groups.

Interventions for improving outcomes in patients with multimorbidity in primary care and community settings

Summary: Evidence from 17 randomized controlled trials suggests that interventions for people with multimorbidity may improve outcomes through changes in care delivery or patient-oriented support. While there was little or no difference in clinical outcomes, mental health outcomes improved and there were modest improvements in medication adherence, patient health behaviors, and provider prescribing behavior quality.
