A network meta-analysis on the efficacy and safety of thermal and nonthermal endovenous ablation treatments

Summary: This study evaluated the mid-term efficacy and safety of thermal and nonthermal endovenous ablation for the treatment of lower limb superficial venous insufficiency. The results showed that radiofrequency ablation (RFA), cyanoacrylate ablation (CAC), and endovenous laser ablation (EVLA) had higher odds of great saphenous vein (GSV) closure compared to mechanochemical ablation (MOCA). Additionally, EVLA had an increased risk of postoperative paresthesia.

Comparative analysis of endovenous laser ablation versus ultrasound-guided foam sclerotherapy for the treatment of venous leg ulcers

Summary: Endovenous thermal and non-thermal therapeutic approaches have become standard for venous insufficiency treatment, but comparative studies in venous leg ulcer patients are scarce. This study compared the efficacy of endovenous laser ablation (EVLA) and ultrasound-guided foam sclerotherapy (UGFS) for venous leg ulcers (VUs). Results showed similar rates of ulcer resolution and healing time after endovenous intervention, but a significantly higher relapse rate in UGFS treated patients.

Summary: This study evaluated the outcomes of symptomatic patients with superficial chronic venous insufficiency treated with PEM or EVLA. The results showed comparable elimination rates of reflux between PEM and EVLA, and PEM was effective for patients with C6 disease. Closure rates were maintained in both groups 36 months after treatment.

Summary: The study compared the effectiveness of high saphenous ligation and stripping, radiofrequency ablation, and subfascial endoscopic perforator surgery for the treatment of active venous ulcers. Results showed that subfascial endoscopic perforator surgery combined with radiofrequency ablation was superior in promoting ulcer healing compared to high saphenous ligation and stripping, and radiofrequency ablation of axial and perforators.

A Contemporary Comparison of Cyanoacrylate, Radiofrequency, and Endovenous Laser Ablation on Healing of Active Venous Ulceration

Summary: This study compares the healing rates and complications of different ablation methods for active lower extremity venous ulcers, and identifies factors that affect successful healing and prevention of recurrence. The results show that cyanoacrylate ablation has longer healing times and more complications compared to other methods. Compliance with compression therapy and working with a wound clinic are predictive of venous ulcer healing.

Healing outcomes and predictors among patients with venous leg ulcers treated with compression therapy

Summary: This study compared the healing outcomes of two layer bandage, four-layer bandage, and compression stockings and found that the duration of compression therapy was an independent predictor of healing. Two layer bandage showed the most favorable healing outcome in the short-term, but the type of compression system used did not make a difference in the healing outcome as time went on, suggesting the need to consider adjuvant therapies at an earlier stage.

A randomized clinical trial of the effects of saphenous and perforating veins radiofrequency ablation on venous ulcer healing (VUERT trial)

Summary: RFA combined with compression therapy is an effective treatment for VLU, showing lower ulcer recurrence rates and faster healing velocity compared to compression therapy alone. Though two participants in the RF group experienced type 1 endovenous heat-induced thrombosis (EHIT), the overall results suggest that RFA plus compression is safe and beneficial in reducing ulcer recurrence and improving clinical outcomes.
