High Prevalence of Spinopelvic Risk Factors in Patients With Post-Operative Hip Dislocations

Summary: This study aimed to investigate the prevalence of standing posterior pelvic tilt (PT), lumbar spine stiffness, low pelvic incidence (PI), and severe sagittal spinal deformity (SSD) in unstable total hip arthroplasty (THA) patients and compare them to a representative control population. The results showed that excessive standing posterior PT, low lumbar flexion, and severe SSD were more prevalent in unstable THAs. Screening for these risk factors preoperatively and planning accordingly may help identify at-risk patients and reduce the prevalence of dislocation.

Summary: In recent years, different alignments have been described for acetabular components in total hip arthroplasty (THA) to reduce the risk of impingement and edge loading. This study compared the risk of impingement and edge loading between mechanical, anatomic, and combined kinematic alignments. The results showed that the combined kinematic alignment significantly reduced the risk compared to mechanical and anatomic alignments.

Prevalence of Risk Factors for Adverse Spinopelvic Mobility Among Patients Undergoing Total Hip Arthroplasty

Summary: In this study, it was found that 46% of patients had spinopelvic pathology driven by one or more risk factors, with 13% exhibiting adverse spinopelvic mobility. The presence of multiple risk factors was positively correlated with the risk of adverse spinopelvic mobility, with 57% of patients with all three risk factors experiencing this complication.

The Effect of a Degenerative Spine and Adverse Pelvic Mobility on Prosthetic Impingement in Patients Undergoing Total Hip Arthroplasty

Summary: This study aimed to quantify the clinical consequences of a degenerative spine and adverse pelvic mobility on prosthetic impingement in patients undergoing total hip arthroplasty. The results showed that patients with limited lumbar flexion, sagittal imbalance, and increased anterior pelvic mobility were more likely to experience anterior impingement, while those with larger posterior pelvic mobility were more prone to posterior impingement. Targeting a patient's functional safe zone rather than a generic target reduced impingement, but some patients still experienced unavoidable impingement.
