Sleep disturbances in HIV infection and their biological basis

Summary: Antiretroviral therapy has reduced morbidity and mortality in people living with HIV, but the development of ageing-related co-morbidities and sleep disturbances has affected their quality of life. Sleep disturbances in PLWH, including insomnia, obstructive sleep apnoea, and poor sleep quality, impair daytime functioning and can contribute to metabolic dysregulation and cardiometabolic disease. The pathogenesis of sleep disturbances in PLWH involves immunological changes, chronic immune activation, and potential side effects of antiretroviral therapy. Understanding the link between HIV, immune activation, and sleep is crucial for minimizing population morbidity and mortality.

Summary: Studies in Africa show that beta-cell dysfunction appears to play a more significant role in diabetes compared to insulin resistance, with a higher risk of prediabetes and diabetes associated with beta-cell dysfunction, insulin resistance, and combined dysfunction and resistance. Further cohort studies are needed to confirm these findings.

Nocturnal dipping of heart rate and blood pressure in people with HIV in Tanzania

Summary: People with HIV have a >2-fold greater risk for cardiovascular disease, which may be associated with abnormalities in 24-h ambulatory blood pressure measurement. Nocturnal non-dipping of both blood pressure and heart rate is more common in HIV-infected individuals, with clinical correlates such as BMI, CD4(+) cell count, C-reactive protein, and markers of renal disease. Further research is needed to determine the causes and consequences of these differences.
