Biomarker Concentrations and Their Temporal Changes in Patients With Myocardial Infarction and Nonobstructive Compared With Obstructive Coronary Arteries: Results From the PLATO Trial

Summary: Investigating biomarker concentrations and their changes can provide new insights into the pathobiology of nonobstructive myocardial infarction (MINOCA). Patients with MINOCA have higher initial inflammatory activity, similar myocardial dysfunction, and faster recovery compared to those with MI with obstructive coronary artery disease.

An Updated Meta-Analysis of the Relationship Between Helicobacter pylori Infection and the Risk of Coronary Heart Disease

Ling Tong et al.

Summary: This meta-analysis demonstrates an association between Helicobacter pylori infection and an increased risk of coronary heart disease (CHD). Further research is needed to investigate whether early eradication of H. pylori can reduce the morbidity of CHD.

Increased soluble urokinase plasminogen activator levels modulate monocyte function to promote atherosclerosis

George Hindy et al.

Summary: This study investigated the pathogenic involvement of soluble urokinase plasminogen activator receptor (suPAR) in atherosclerosis. The researchers found that suPAR levels were predictive of coronary artery calcification and cardiovascular events. Through genetic analysis and experimental models, they identified a variant in the PLAUR gene that led to higher suPAR levels and confirmed a causal association between suPAR levels and atherosclerotic phenotypes. The study also found that suPAR affected monocyte function, leading to increased inflammation and chemotaxis.

Clinical, genetic, and experimental increase in soluble urokinase plasminogen activator receptor levels promotes atherosclerosis

George Hindy et al.

Summary: The study using epidemiologic, genetic, and experimental approaches found that suPAR is a pathogenic factor for atherosclerosis, with its levels strongly associated with cardiovascular outcomes and predictive of coronary artery calcification. The missense variant rs4760 in the PLAUR gene leads to higher suPAR levels, and Mendelian randomization analysis confirms a causal relationship between genetically predicted suPAR levels and atherosclerotic phenotypes. In experimental models, increased suPAR levels modulate monocyte function to promote atherosclerosis.

Summary: Inflammation is closely linked to atherosclerosis, and recent clinical trials have shown the effectiveness of anti-inflammatory interventions in improving outcomes for patients with the disease. Further research is needed to optimize these interventions and improve patient selection.

Summary: This study reveals a significant association between IL-6 binary complex concentration and future myocardial infarction (MI), and suggests that plasma IL-6 and IL-6 binary complex concentration may partly explain the increased risk of MI in smokers and hypertensive participants.

Summary: Multimodality imaging with coronary OCT and CMR identified potential mechanisms in 84.5% of women with a diagnosis of MINOCA, 75.5% of which were ischemic and 24.5% of which were nonischemic, providing valuable guidance for medical therapy in secondary prevention.

Diagnostic pathways in myocardial infarction with non-obstructive coronary artery disease (MINOCA)

Giovanni Occhipinti et al.

Summary: This narrative review discusses the downstream diagnostic approaches to MINOCA and emphasizes the possibility of establishing the underlying cause of MINOCA in the vast majority cases. The exclusion of other possible causes for cardiac troponin elevation has notable implications for tailored secondary prevention measures aimed at improving the overall prognosis of AMI.
