While antipsychotics are effective for positive symptoms, they are not efficacious for negative and cognitive symptoms. This review discusses the dysfunction of various metabolic pathways in different brain regions and suggests that deficits in bioenergetic subprocesses are a core feature of schizophrenia. Pharmacologic and nonpharmacologic interventions targeting these pathways are discussed as potential ways to improve symptoms.

Summary: Treatment and prevention strategies for schizophrenia require understanding the underlying mechanisms. Increasing evidence suggests a redox imbalance in schizophrenia patients, with altered antioxidant functions and increased oxidative damage to lipids in the blood. Decreased levels of non-enzymatic antioxidants and correlations between blood oxidative stress levels and symptom severity have been observed. Adjunct treatment of antipsychotics with the antioxidant N-acetyl cysteine appears to be effective. Further studies are needed to assess its efficacy as a preventive strategy.
