Modulation of post-pacing action potential duration and contractile responses on ventricular arrhythmogenesis in chloroquine-induced long QT syndrome

Summary: This study aimed to investigate the arrhythmogenic role of excitation-contraction coupling in the right ventricular outflow tract in chloroquine-induced long QT syndrome. The results showed that sequential infusion of chloroquine and azithromycin triggered ventricular tachycardia, which could be suppressed by NCX inhibitors or MgSO4.

An inherited sudden cardiac arrest syndrome may be based on primary myocardial and autonomic nervous system abnormalities

Summary: A recently discovered sudden cardiac arrest (SCA) syndrome is associated with a high-risk haplotype containing the DPP6 gene. This study found that carriers of the risk haplotype have functional changes in both the heart and autonomic nervous system, which may contribute to ventricular fibrillation (VF). These findings suggest that the primary abnormalities in SCA syndrome may reside in both the heart and the autonomic nervous system.

Summary: A novel somatic mutation in the GNAS gene associated with RVOT tachycardia was identified, leading to constitutive activation of G(s)alpha, increased intracellular cAMP levels, enhanced L-type calcium currents, and triggered activity. These findings suggest that signal transduction proteins with somatic mutations can play a role in the development of RVOT tachycardia through modulation of cAMP and downstream effectors.

Calcium dysregulation increases right ventricular outflow tract arrhythmogenesis in rabbit model of chronic kidney disease

Summary: Chronic kidney disease (CKD) increases the risk of arrhythmia by affecting calcium dysregulation in the right ventricular outflow tract (RVOT). CKD rabbits with RVOT tissues or cardiomyocytes showed abnormalities in action potential duration, sustained VT incidence, and calcium current levels, which may contribute to the induction of arrhythmias. Treatment with specific inhibitors attenuated the VT in CKD RVOT tissues, indicating a potential therapeutic target for managing arrhythmias in CKD.

Summary: Idiopathic ventricular arrhythmias are ventricular tachycardias or premature ventricular contractions that are presumably not related to myocardial scar or disorders of ion channels. Ventricular arrhythmias without underlying structural heart disease often arise from the ventricular outflow tracts, with the right ventricular outflow tract being the most common site of origin. Radiofrequency catheter ablation is an effective and safe treatment strategy for these focal ventricular arrhythmias.
