Early versus delayed penile prosthesis insertion for refractory ischemic priapism

Baher Salman et al.

Summary: This study compared the outcomes of early versus delayed penile prosthesis insertion in patients with refractory ischemic priapism. Results showed higher postoperative complications in the early insertion group and higher intraoperative complications in the delayed insertion group, likely due to corporal fibrosis. Early penile prosthesis insertion was found to be a safer and more effective treatment option compared to delayed insertion.

Summary: Ischemic priapism, the most common type of priapic episode, requires prompt treatment to prevent tissue necrosis. If conservative measures fail or recurrence occurs, surgical intervention, including penile prosthesis implantation, may be the best solution.

Surgical and minimally invasive treatment of ischaemic and non-ischaemic priapism: a systematic review by the EAU Sexual and Reproductive Health Guidelines panelt

U. Milenkovic et al.

Summary: Surgical treatments for ischemic priapism (IP) include shunts or penile implants, while embolisation may be required for non-ischemic priapism (NIP). A systematic review found acceptable success rates for shunts in IP, with the best outcomes seen with penile prosthesis implantation in cases exceeding 48 hours. Embolisation is the mainstay technique for NIP, providing high resolution rates and adequate erectile function.

Conservative and medical treatments of non-sickle cell disease-related ischemic priapism: a systematic review by the EAU Sexual and Reproductive Health Panel

Paolo Capogrosso et al.

Summary: Conservative and medical treatments are the first step in managing ischemic priapism, but their efficacy is not clear. Our systematic review found that intracavernous injection with sympathomimetic (ICIs) agents was the most frequently utilized treatment, with efficacy ranging from 0 to 100%.

Immediate insertion of a soft penile prosthesis as a new option for a safe and cost-effective treatment of refractory ischemic priapism

Franco Palmisano et al.

Summary: Immediate insertion of a soft penile prosthesis (sPP) in the management of refractory ischemic priapism (IP) provides immediate pain relief, preservation of sexual function and penile size, with higher surgery reproducibility in emergencies. Additionally, potential reduction in financial and resource burdens of IP on the health-care system.
