Vitamin-K-antagonist phenprocoumon versus direct oral anticoagulants in patients with atrial fibrillation: a real-world analysis of German claims data

Lisette Warkentin et al.

Summary: This retrospective observational study in Germany compared the effectiveness and safety of DOACs and phenprocoumon in patients with atrial fibrillation. The results showed similar outcomes between the two treatments, with only a few statistically significant differences in bleeding risk and risk of death.

Usefulness of transesophageal echocardiography before cardioversion in atrial arrhythmias

Katarzyna Kosmalska et al.

Summary: Among 269 patients with persistent atrial fibrillation or atrial flutter undergoing transesophageal echocardiography before cardioversion, 29% were found to have left atrial appendage thrombus and/or sludge. Risk factors for the presence of thrombus and/or dense spontaneous echo contrast included signs of dementia, low velocities in left atrial appendage, presence of spontaneous echo contrast in left atrium, consecutive episodes of atrial fibrillation, and longer duration of arrhythmia. Presence of thrombus was independent of the CHA2DS2VASc score in patients with a score > 1.

Antithrombotic treatmentmanagement in low stroke risk patients undergoing cardioversion of atrial fibrillation < 48 h duration: results of an EHRA survey

Federico Migliore et al.

Summary: The survey results showed heterogeneity in anticoagulation management before and after cardioversion in low stroke-risk patients with atrial fibrillation duration <48 hours. Periprocedural low-molecular-weight heparin was used more than non-vitamin K antagonist oral anticoagulants, and there was higher utilization of pre-cardioversion transesophageal echocardiography for electrical cardioversion than for pharmacological cardioversion.

How lower doses of direct oral anticoagulants are interpreted in clinical practice: a national survey of the Italian Atherosclerosis, Thrombosis and Vascular Biology (ATVB) Study Group

Andrea Rubboli et al.

Summary: The interpretation of the lower doses of DOAC in current Italian clinical practice appears largely correct and shared. However, there are some remaining uncertainties, especially regarding dabigatran, indicating the need for continuous educational efforts.
