Invasive Scedosporium and Lomentospora prolificans Infections in Australia: A Multicenter Retrospective Cohort Study

Summary: Higher mortality and treatment failure rates were observed in Lomentospora prolificans infections or in highly immunosuppressed patients with hematological malignancy or hematopoietic stem cell transplant. Adjunctive surgery was associated with lower mortality and decreased odds of 1-month treatment failure. This study described the predisposing factors, clinical manifestations, and outcomes of Scedosporium/Lomentospora prolificans infections, and identified predictors of early and late mortality and treatment failure.

PCR testing is recommended for infections caused by Aspergillus, Candida, and Pneumocystis jirovecii, and combination with serological testing can enhance the diagnosis. Molecular diagnostics represent a non-classical approach for other IFD and have improved confidence in testing through continued validation and standardization. Molecular testing can also diagnose antifungal resistance, and next-generation sequencing has the potential to improve our understanding of fungal phylogeny, epidemiology, and resistance mechanisms.
