Evaluating the Benefits and Risks of Ex Vivo Liver Resection and Autotransplantation in Treating Hepatic End-stage Alveolar Echinococcosis

Yiwen Qiu et al.

Summary: This study analyzed the benefits and risks of ex vivo liver resection and autotransplantation (ELRA) in end-stage hepatic alveolar echinococcosis (AE). The long-term overall survival of the ELRA group was superior to that of the nonsurgical group, and patients with a predicted 12-month mortality risk >75% would significantly benefit from ELRA.

Ex Vivo Liver Resection and Autotransplantation Should It be Used More Frequently?

Joshua Weiner et al.

Summary: This study advocates for the wider utilization of ex vivo liver resection techniques for otherwise unresectable liver tumors. The results show favorable survival rates and demonstrate that the required skillset for ex vivo resection is widespread, with fewer complications than previously assumed.

Current indications of ex-situ liver resection: A systematic review

Alejandro Serrablo et al.

Summary: Ex situ liver resection and autotransplantation is a valuable option for patients with conventionally unresectable hepatic tumors and normal liver function. However, there are risks of adverse events and postoperative recurrences. Tumor size and resection range are correlated with surgical outcomes.

The technical aspects of ex vivo hepatectomy with autotransplantation: a systematic review and meta-analysis

Maria Baimas-George et al.

Summary: This study reviewed the technical variations of ex vivo hepatectomies in the literature and found that there is a split between using venovenous bypass (VVB) and temporary portacaval shunts (PCS) for the procedure. However, intraoperative variables and morbidity were not significantly different between the two groups, but patients with temporary PCS had significantly lower 90-day mortality. Further research and a consensus statement are needed to better understand these differences between technical approaches.

Vascular surgery in liver resection

Olga Radulova-Mauersberger et al.

Summary: Vascular surgery plays a crucial role in oncological liver resections, particularly for malignant liver tumors with vascular involvement. Surgery remains the only treatment option for providing long-term survival in advanced hepatic malignancy.
