A call for action: Recommendations to improve transition to adult care for youth with complex health care needs

Alene Toulany et al.

Summary: Youth with complex health care needs often receive care from pediatricians, but the age at which they transition to adult care varies, creating barriers to continuity of care. Collaboration between pediatric and adult care providers is necessary to streamline and protect the transition process for these youth across different care settings.

Association between transition readiness and mental health comorbidity in youth with chronic health conditions

Brooke Allemang et al.

Summary: In this study, the transition readiness of youth with co-occurring chronic health and mental health conditions was examined using the Transition Readiness Assessment Questionnaire (TRAQ). The presence of a mental health comorbidity was found to be associated with higher overall TRAQ scores. Older age and female gender were also associated with greater transition readiness. Future research should further investigate why these youth exhibit higher transition readiness.
