Hyperactive Distal Colonic Motility and Recovery Patterns Following Right Colectomy: A High-Resolution Manometry Study

Cameron I. I. Wells et al.

Summary: This study used high-resolution manometry to evaluate distal colonic motility after right-sided colectomy. The results showed that hyperactive cyclic motor patterns emerged in the distal colon after surgery, occupying 81.8% of the recording. These patterns gradually resolved during the first 4 postoperative days. Other abnormal repetitive hyperactive patterns may contribute to prolonged ileus development.

Asia-Pacific guidelines for managing functional dyspepsia overlapping with other gastrointestinal symptoms

Kok-Ann Gwee et al.

Summary: Contemporary systems categorize patients into sub-classes based on symptoms to enable targeted treatment, but overlapping symptom categories negatively impact outcomes, lacking guidance on management. An APAGE working group developed clinical practice guidelines for management of patients with functional dyspepsia (FD) who have overlaps with other functional gastrointestinal disorders. They identified putative pathophysiology and provided a management algorithm for primary and secondary care clinicians.

Associations between postprandial symptoms, hydrogen and methane production, and transit time in irritable bowel syndrome

Joost P. Algera et al.

Summary: This study investigates the associations between oronal transit time (OATT), rectal sensitivity and the lactulose nutrient challenge test (LNCT) in patients with irritable bowel syndrome (IBS). The study findings suggest that rectal sensitivity is associated with abdominal pain, while faster OATT is associated with more severe abdominal discomfort, rumbling, nausea, and urgency. The study also highlights the importance of OATT in hydrogen and methane production, as well as its correlation with postprandial symptoms and gas concentrations.

Gut-brain axis dysfunction underlies FODMAP-induced symptom generation in irritable bowel syndrome

Jie Wu et al.

Summary: Fructans induce similar changes in small bowel motility and colon gas and volume in patients with irritable bowel syndrome (IBS) and healthy controls (HCs). Increased symptom responses to fructans in IBS are associated with altered brain responses in pain-related regions, indicating a dysregulation of the gut-brain axis in FODMAP-induced symptom generation in IBS.

AGA Clinical Practice Update on the Role of Diet in Irritable Bowel Syndrome: Expert Review

William D. Chey et al.

Summary: Irritable bowel syndrome (IBS) is a common gastrointestinal disorder that affects quality of life. The latest clinical practice update recommends dietary modifications, such as the low-FODMAP diet, as the primary treatment for IBS symptoms. Best practice advice includes involving patients who are insightful and motivated in dietary changes, avoiding restrictive diets for patients who are poor candidates, highlighting the benefits of soluble fiber and the low-FODMAP diet, and following a three-phase approach for the low-FODMAP diet. The efficacy of a gluten-free diet for IBS is uncertain and there is limited evidence to support the routine use of biomarkers in clinical practice.
