Evidence or Prejudice? Critical Re-Analysis of Randomized Controlled Trials Comparing Overall Survival After Cisplatin Versus Carboplatin-Based Regimens in Advanced Urothelial Carcinoma

Anke Richters et al.

Summary: Guidelines have recommended cisplatin for the treatment of advanced urothelial carcinoma since 2008, but a secondary analysis of individual patient data did not show an overall survival benefit compared to carboplatin. Considering the lower toxicity and larger patient eligibility for carboplatin, a reevaluation of guideline recommendations is warranted.

Chemotherapy is superior to checkpoint inhibitors after radical surgery for urothelial carcinoma: a systematic review and network meta-analysis of oncologic and toxicity outcomes

Ekaterina Laukhtina et al.

Summary: The objective of this study was to compare the oncologic and toxicity outcomes of adjuvant immunotherapy with immune checkpoint inhibitors (ICIs) and adjuvant chemotherapy for UC patients. The analysis showed that adjuvant chemotherapy had better results in both bladder UC and UTUC compared to ICIs, and the risk of adverse events with ICIs was comparable to that of observation/placebo.

Contemporary patterns of presentation, diagnostics and management of upper tract urothelial cancer in 101 centres: the Clinical Research Office of the Endourological Society Global upper tract urothelial carcinoma registry

Joyce Baard et al.

Summary: A multicentre registry was launched to assess patterns of presentation, diagnostics, and treatment in patients with upper tract urothelial carcinoma (UTUC). Data from 2380 patients over a 5-year period were collected, showing that most patients presented with symptoms, with macroscopic hematuria being the main symptom. Computed tomography (CT) was the most common imaging modality used, and ureteroscopy (URS) was part of the diagnostic process for nearly half of the patients, with a significant portion receiving endoscopic treatment. Overall, the data aligns with known epidemiologic characteristics of UTUC, highlighting the preference for CT imaging and the increasing use of diagnostic URS, though a substantial number of treatment decisions were made without complete endoscopic information.

Update on systemic treatment of upper tract urothelial carcinoma: a narrative review of the literature

Summary: Urothelial cancer, commonly found in the urinary bladder and upper urinary tract, faces challenges in diagnosis, clinical staging, and recurrence post-surgery. The expanding therapeutic options, including novel targeted therapies and biomarker research, are reshaping the treatment landscape for UC. However, systemic treatment of UTUC presents specific challenges, such as loss of renal function after RNU affecting chemotherapy eligibility.

Molecular classification and diagnostics of upper urinary tract urothelial carcinoma

Yoichi Fujii et al.

Summary: Through an integrated genetic study involving 199 UTUC samples, the landscape of genetic alterations in UTUC was delineated, enabling genetic/molecular classification. UTUC is classified into five subtypes based on the mutational status of TP53, MDM2, RAS, and FGFR3, with discrete profiles of gene expression, tumor location/histology, and clinical outcome. Sequencing of urine sediment-derived DNA has high diagnostic value for UTUC with 82.2% sensitivity and 100% specificity, providing a solid basis for better diagnosis and management of UTUC.
