Pulmonary important outcomes after extremely preterm birth: Parental perspectives

Summary: The aim of this study was to describe pulmonary important outcomes (PIO) reported by parents of extremely preterm children. Parents of children aged 18 months to 7 years, born at less than 29 weeks' gestational age, were surveyed over the course of one year. The proportion of parents describing PIO and the themes they mentioned were examined. The results showed that 44% of parental responses mentioned PIO and discussed 24 themes related to NICU hospitalization and long-term respiratory health. PIO were more likely to be mentioned in males and among children who were readmitted for respiratory problems.

Preterm birth and asthma and COPD in adulthood: a nationwide register study from two Nordic countries

Postnatal corticosteroid use for prevention or treatment of bronchopulmonary dysplasia in England and Wales 2012-2019: a retrospective population cohort study

Summary: This study describes the population of babies who do and do not receive postnatal corticosteroids for the prevention or treatment of bronchopulmonary dysplasia (BPD) in England and Wales. It found that approximately 1 in 12 babies born less than 32 weeks and 1 in 4 born less than 27 weeks received postnatal corticosteroids for BPD. Dexamethasone was the most commonly used corticosteroid, and its use increased over time.

Summary: Early systemic postnatal corticosteroid treatment can prevent bronchopulmonary dysplasia (BPD) and the combined outcome of mortality or BPD, but it may increase risks of gastrointestinal perforation, cerebral palsy, and the combined outcome of mortality or cerebral palsy. Most beneficial and harmful effects are related to early treatment with dexamethasone rather than hydrocortisone, although hydrocortisone may help prevent mortality. Long-term follow-up into late childhood is crucial for assessing the impact of early corticosteroid treatment on higher-order neurological functions. Further research on early corticosteroids, especially hydrocortisone, should prioritize longer-term survival free of neurodevelopmental disability as the primary outcome.
