Orthostatic Hypotension and Antiparkinsonian Drugs: A Systematic Review and Meta-analysis

Summary: In Parkinson's disease, orthostatic hypotension (OH) is multifactorial and can be exacerbated by antiparkinsonian medication, increasing the risk of falls, weakness, and fatigue. A systematic review and meta-analysis of randomized controlled trials (RCTs) was conducted to assess the association between antiparkinsonian drugs and OH, with findings indicating that MAO-B inhibitors and dopamine agonists were not significantly associated with increased risk of OH when compared to placebo. However, most studies did not specifically report OH, highlighting the need for future trials to address this issue.

Summary: The review found that initial treatment with levodopa provides better motor benefits compared to dopamine agonists, but is also more likely to cause motor complications. There was little evidence to suggest that any particular formulation or method of administration of dopamine agonists is superior. Long-acting forms of levodopa and combination formulations with entacapone do not appear to differ in efficacy from immediate-release levodopa for early disease motor symptoms. Patients using dopamine agonists may have a higher risk of impulse control disorders compared to those using levodopa.

A prospective study showed that PD patients had mild sympathetic impairment at first evaluation, which worsened with disease progression. Some patients with orthostatic symptoms did not have low blood pressure during testing.
