Non-invasive stereotactic arrhythmia radiotherapy for ventricular tachycardia: results of the prospective STARNL-1 trial

Martijn H. van der Ree et al.

Summary: This study aims to evaluate the efficacy and safety of Stereotactic Arrhythmia Radiotherapy (STAR). It found that STAR can reduce the number of treated ventricular tachycardia episodes by more than 50% in 67% of patients with therapy-refractory VT. No decrease in cardiac and pulmonary function or treatment-related serious adverse events were observed during follow-up.

Summary: The STOPSTORM consortium, funded by the EU Horizon 2020 Framework, aims to establish a standardized treatment and outcome platform for investigating STAR in ventricular tachycardia. The consortium consists of 31 clinical and research institutions and is divided into nine work packages. A comprehensive questionnaire was conducted to review the current clinical practice of STAR in Europe, highlighting potential areas of optimization and harmonization.

Management of ventricular tachycardia in patients with ischaemic cardiomyopathy: contemporary armamentarium

Ann-Kathrin Kahle et al.

Summary: Sudden cardiac death caused by ischaemic cardiomyopathy is a major concern, with implantation of cardioverter-defibrillator being a common preventive measure. Catheter ablation has been shown to reduce ventricular tachycardia recurrences in patients with ICM, and emerging concepts like sympathetic cardiac denervation hold promise for expanding therapeutic options in therapy-refractory VT. Further research is needed to investigate the potential benefits of these novel strategies for improving patient outcomes.

Impact of High-Dose Irradiation on Human iPSC-Derived Cardiomyocytes Using Multi-Electrode Arrays: Implications for the Antiarrhythmic Effects of Cardiac Radioablation

Jae Sik Kim et al.

Summary: Cardiac radioablation is an effective treatment for refractory ventricular arrhythmias. This study investigated the acute effects of high-dose radiation on human cardiomyocytes and found that it immediately and reversibly modifies the electrical conduction of the cells. The study also suggests that compensatory mechanisms at the cellular level may contribute to the immediate antiarrhythmic outcome of cardiac radioablation.

Summary: This retrospective study aimed to evaluate the clinical outcomes of SBRT in patients with ES. The results showed that SBRT was associated with a significant reduction in VT burden in patients with refractory VT. However, further prospective randomized control trials are needed to validate these findings.

Early Changes in Rat Heart After High-Dose Irradiation: Implications for Antiarrhythmic Effects of Cardiac Radioablation

Myung-Jin Cha et al.

Summary: High-dose cardiac radioablation induced mononuclear cell-related intracellular and extracellular edema, diffuse vacuolization, and widened intercalated discs in rat hearts within 1 month. ECG showed intracardiac conduction delay and significant ST-segment changes. These findings suggest that early antiarrhythmic effects following cardiac radioablation are due to conduction disturbances and membrane potential alterations, rather than necrosis.
