Not surgical technique, but etiology, contralateral MRI, prior surgery, and side of surgery determine seizure outcome after pediatric hemispherotomy

Georgia Ramantani et al.

Summary: This study aimed to evaluate the determinants of seizure outcome following pediatric hemispherotomy. Through a retrospective analysis of 457 children who underwent hemispheric surgery, variables such as acquired etiology, hemimegalencephaly, contralateral MRI findings, prior resective surgery, and left hemispherotomy were identified as significant determinants of seizure recurrence. The study found no significant difference in seizure-freedom rates between vertical and horizontal hemispherotomy techniques.

Hemispherectomy Outcome Prediction Scale: Development and validation of a seizure freedom prediction tool

Alexander G. Weil et al.

Summary: A predictive model for seizure freedom in children undergoing cerebral hemispheric surgery was developed and validated using data from 1267 surgeries across multiple centers and countries. Factors such as age, age at seizure onset, metabolic factors were found to be predictive of seizure freedom after surgery. The Hemispheric Surgery Outcome Prediction Scale (HOPS) may be useful for clinical decision-making.
