Transmission of clones of carbapenem-resistant Escherichia coli between a hospital and an urban wastewater treatment plant*

Molecular epidemiology and genomic insights into the transmission of carbapenem-resistant NDM-producing Escherichia coli

Summary: This study identified 27 carbapenem-resistant Escherichia coli (CREC) strains, of which 17 carried the blaNDM-5 gene. All 23 NDM-producing E. coli strains were resistant to all antibiotics except tigecycline, colistin, and cefiderocol. Furthermore, two copies of the blaNDM-5 gene were found on a single plasmid, highlighting the need for attention.

High prevalence of Escherichia coli co-harboring conjugative plasmids with colistin- and carbapenem resistance genes in a wastewater treatment plant in China

Summary: This study investigated the occurrence and characteristics of carbapenem and colistin-resistant Escherichia coli (CCREC) in wastewater and sludge samples from a urban wastewater treatment plant in Jinan, China. The study found that 8 out of 168 samples tested positive for CCREC, with high prevalence of multidrug resistance and the presence of virulence genes. The study also observed that the resistance genes could be transferred through conjugation, indicating the potential for further dissemination of resistant E. coli.

Genomic characterization of high-risk Escherichia coli and Enterobacter hormaechei clones recovered from a single tertiary-care hospital in Pakistan

Summary: The presence of extended-spectrum beta-lactamase (ESBL)-producing Enterobacterales in urinary tract infection cases in Pakistan was investigated. The study revealed the presence of ESBL-producing Escherichia coli and Enterobacter hormaechei strains, some of which were multidrug-resistant and carried resistance genes. Screening of these strains may be effective in controlling the spread of multidrug-resistant bacteria.

Phenotype-genotype correlations among carbapenem-resistant Enterobacterales recovered from four Egyptian hospitals with the report of SPM carbapenemase

Summary: This study characterized carbapenem resistance among Enterobacterales isolates in Egypt and found a high prevalence of carbapenem non-susceptibility. The most common carbapenemase encoding gene was bla(NDM), followed by bla(VIM). More than half of the isolates co-harbored multiple carbapenemase-encoding genes. Different genotypes had a significant impact on meropenem MIC values.

Dissemination of blaNDM-5 in Escherichia coli through the IncX3 Plasmid from Different Regions in China

Summary: This study investigated the dissemination of the bla(NDM-5) gene in carbapenem-resistant Escherichia coli isolates from different regions in China. Seven bla(NDM-5)-positive E. coli strains were detected, which showed resistance to multiple antibiotics and carried other antimicrobial resistance genes. The bla(NDM-5) gene was located on 46 kb IncX3 plasmids in all isolates. This study confirms the widespread dissemination of bla(NDM-5)-carrying plasmids in E. coli isolates in China, highlighting the need for stringent surveillance and control measures.

Tracking Klebsiella pneumoniae carbapenemase gene as an indicator of antimicrobial resistance dissemination from a hospital to surface water via a municipal wastewater treatment plant

Summary: This study traced the presence of the antimicrobial resistance gene KPC and the viable organisms carrying this gene in hospital wastewater, municipal wastewater treatment plants (WWTP), and receiving water samples. The study found high concentrations of the KPC gene and the viable organisms in the hospital wastewater and the initial stages of the WWTP, but not in the final effluent following UV disinfection. The KPC-producing organisms were also found in the biosolids throughout the WWTP, but not in the processed compost-products made from the biosolids.

Emergence of blaNDM-1, blaNDM-5, blaKPC-2 and blaIMP-4 carrying plasmids in Raoultella spp. in the environment

Summary: This study investigated the distribution and resistance mechanisms of carbapenem-resistant Raoultella spp. (CRR) in a wastewater treatment plant (WWTP) in eastern China. The results showed that CRR were mainly found in the water inlet and mud cake stacking area of the WWTP, with a higher detection rate in the water inlet. All CRR isolates were resistant to multiple antibiotics and various carbapenemase genes were identified. Interestingly, R. ornithinolytica isolates from the WWTP were closely related to those from human samples in China. Plasmid analysis revealed the transmission routes and core structures of carbapenemase genes in the environment.

Characterization of blaNDM-5-and blaCTX-M-199-Producing ST167 Escherichia coli Isolated from Shared Bikes

Summary: Shared bikes can serve as potential carriers of antimicrobial resistant bacteria and genes. This study analyzed the whole genome sequences of 24 Escherichia coli strains isolated from shared bikes in Beijing. The results showed that some strains carried multiple resistance genes and various types of plasmids. Two strains exhibited high similarities in core genomes and plasmid profiles with strains from humans and animals. These findings provide valuable information for preventing and controlling the spread of antimicrobial resistant bacteria between animals, humans, and the environment.

Genomic Epidemiology of Global Carbapenemase-Producing Escherichia coli, 2015-2017

Summary: This study describes the global molecular epidemiology of carbapenemase-producing Escherichia coli from 36 countries during 2015-2017. The most common carbapenemases identified were oxacillinase (OXA) 181, New Delhi metallo-beta-lactamase (NDM) 5, OXA-48, Klebsiella pneumoniae carbapenemase 2, and NDM-1. The study identified 5 dominant sequence types, with 4 being global and 1 limited to Turkey. Additionally, the study found nearly identical plasmids carrying the OXA-181 gene in different sequence types and countries. The findings highlight the diverse and geographically varied nature of carbapenemase-producing E. coli strains worldwide.

Summary: Extended-spectrum beta-lactamase (ESBL) and carbapenemase-producing Enterobacterales are still present in the environment even after conventional treatment in wastewater treatment plants. Enteric opportunistic pathogens such as Klebsiella pneumoniae and Acinetobacter baumannii are efficiently removed, but Escherichia coli and Enterococcus spp. still remain in relatively high concentrations in the effluent. Improvement in wastewater treatment technologies is necessary to minimize the risk of environmental contamination and public health issues related to ESBL and carbapenemase-producing Enterobacterales.
