Validity evidence for observational ERCP competency assessment tools: a systematic review

Summary: This study systematically evaluated the validity evidence of observational assessment tools for endoscopic retrograde cholangiopancreatography (ERCP). The BESAT, ERCP DOPS, and TEESAT showed strong validity evidence compared with other assessments. Integrating these tools into training may facilitate learners’ development and support competency decision making.

The results showed that personal attributes, such as honesty, team player skills, and self-awareness, were highly valued by the training program directors/experts. However, there is room for improvement in the current application process, as the satisfaction level was moderate.

Mechanical simulators, due to their precision, safety, and repeatability, are recommended for training, although more evidence is needed. Sensors in simulators could provide detailed feedback, and rapid prototyping and 3D printing may be used to create customized training models.
