Incidence and predictors of remission and relapse of type 2 diabetes mellitus in Japan: Analysis of a nationwide patient registry

Summary: This study aimed to determine the incidence of remission and 1-year relapse in patients with type 2 diabetes and associated factors. The overall incidence of remission in Japanese patients was 10.5 per 1000 person-years, with higher rates for those with HbA1c levels of 48 to 53 mmol/mol (6.5% to 6.9%), those taking no glucose-lowering drugs at baseline, and those with a =10% BMI reduction in 1 year. Shorter duration, lower baseline HbA1c, higher baseline BMI, greater BMI reduction at 1 year, and no glucose-lowering drugs at baseline were significantly associated with remission. Among those with remission, approximately two-thirds relapsed within 1 year. Longer duration, lower baseline BMI, and lower BMI reduction at 1 year were significantly associated with relapse.

Metabolic bariatric surgery as a therapeutic option for patients with type 2 diabetes: A meta-analysis and network meta-analysis of randomized controlled trials

Summary: This network meta-analysis compared different types of metabolic surgery with non-surgical therapy for the treatment of type 2 diabetes. The results showed that metabolic bariatric surgery (MBS) was more effective in reducing HbA1c, achieving diabetes remission, and reducing BMI compared to medical therapy. One-anastomosis gastric bypass (OAGB) and sleeve gastrectomy (SG) were found to significantly reduce HbA1c levels. All surgical procedures (except for greater curvature plication) were associated with a significant increase in complete diabetes remission compared to non-surgical therapy. Bilio-pancreatic diversion (BPD), Roux-en-Y gastric bypass (RYGB), and OAGB were also associated with a significant reduction in BMI.

Factors associated with relapse of type 2 diabetes mellitus after laparoscopic sleeve gastrectomy in Japanese subjects: a subgroup analysis of J-SMART study

Summary: A study on 322 Japanese subjects showed that 83.0% of the patients achieved HbA1c level <7% after LSG surgery, but 26.8% experienced diabetes relapse. Preoperative HbA1c, weight loss after surgery, and weight regain were identified as important factors contributing to diabetes relapse.

Summary: The incidence of remission of type 2 diabetes is generally low in routine care settings, but it may be a reasonable goal for patients who lose a significant amount of weight shortly after diagnosis. Factors associated with higher odds of remission include shorter duration of diagnosis, non-prescription of glucose-lowering medication, lower HbA(1c) and BMI levels, White ethnicity, female sex, and lower socioeconomic deprivation.
