The Role of Multifidus in the Biomechanics of Lumbar Spine: A Musculoskeletal Modeling Study

Summary: This study investigated the role of multifidus in the lumbar spine biomechanical model and the impact of asymmetric multifidus atrophy on the biomechanics of the lumbar spine. The results demonstrated that incorporating multifidus in the musculoskeletal model is important for increasing the success rate of simulation and decreasing the overestimation of compressive load on the lumbar spine. Asymmetric multifidus atrophy has negligible effect on the lower lumbar spine in the trunk flexion posture.

Design of a functionally graded porous uncemented acetabular component: Influence of polar gradation

Summary: This study aims to design a novel functionally graded porous metal-backed (FGPMB) acetabular component based on numerical analysis of the mechanical behavior of acetabular components under common failure scenarios and various daily activities and implant-bone interface conditions. Both radial and polar functional gradations were implemented, and the effects of the polar gradation exponent on the failure criteria were evaluated. The FGPMB showed lesser strain-shielding in cancellous bone compared to solid metal-backing, and had a viable alternative to the solid acetabular component, with a gradation exponent of 0.25 appearing to be the most appropriate design parameter.

Numerical Evaluations of an Uncemented Acetabular Component in Total Hip Arthroplasty: Effects of Loading and Interface Conditions

Summary: Using finite element models, this study analyzed the effects of musculoskeletal loading and implant-bone interface conditions on the preclinical analysis of an uncemented acetabular component after total hip arthroplasty. Different loading conditions simulating daily activities and interface conditions were tested. The study found that higher press-fit resulted in increased strains in the pelvic cortex, while strain shielding was higher for the bonded condition during sitting up activity. The study emphasizes the importance of considering variations in loading and interface conditions in the evaluation of uncemented acetabular components.

Bone Remodeling Around Solid and Porous Interbody Cages in the Lumbar Spine

Summary: This study investigated the effects of interbody cages on load transfer and bone remodeling using three-dimensional models. The study found that variations in cage porosity affected peri-prosthetic stress distributions and bone remodeling around the cages. Compared to solid cages, porous cages decreased stresses and strains in the cancellous bone, but increased the range of motion. The increase in bone density predicted in the solid cages may influence disk degeneration in the adjacent segment. An interbody cage with 65-78% porosity could be a viable alternative, provided sufficient mechanical strength is offered.
