The oblique flankplasty is a surgical procedure aimed at narrowing the waist and flank. It is suitable for patients who are primarily concerned with trunk contouring and achieving a narrow waist, as well as having prominent flank rolls. The procedure also aims to better define the upper border of the buttock and improve upper abdominal laxity.
As described previously, the oblique flankplasty is a natural progression of truncal body contouring procedures aimed at narrowing the waist and flank. The ideal patient for oblique flankplasty is one who is primarily interested in contouring of the trunk, achieving the narrowest possible waist, and has prominent flank rolls. Several secondary goals of the oblique flankplasty include better defining the upper border of the buttock and improvement in horizontal upper abdominal laxity. Finally, there are several important areas that are less impacted by the oblique flankplasty, including the medial thigh and buttock. While most dramatic improvement in the overall appearance of the flanks can be achieved, the tradeoff is high riding posterior incisions to create a natural, concave flank. Although the incisions are visible, they are away from the buttock, which helps maintain lower back aesthetics and allows for independent treat-ment of the buttock with fat grafting.
