Development of High-Risk Geriatric Polypharmacy Electronic Clinical Quality Measures and a Pilot Test of EHR Nudges Based on These Measures

Stephen D. Persell et al.

Summary: The study developed and validated seven measures of high-risk polypharmacy and pilot tested electronic health record (EHR)-based nudges grounded in behavioral science. Results showed that the EHR alerts were effective in promoting physicians to address high-risk polypharmacy, with a high response rate to reminders and justifications nudges. Further testing of these interventions is warranted.

Community-based pharmacists' role in deprescribing: A systematic review

Iva Buzancic et al.

Summary: Community-based pharmacists play a crucial role in providing continuing care for chronic multi-morbid patients, and their involvement in deprescribing interventions has been found to be successful in discontinuing medications. Different types of interventions led by community-based pharmacists result in greater medication discontinuation compared to usual care, but the short follow-up periods limit assessment of long-term sustainability.

Deprescribing Education vs Usual Care for Patients With Cognitive Impairment and Primary Care Clinicians The OPTIMIZE Pragmatic Cluster Randomized Trial

Elizabeth A. Bayliss et al.

Summary: This study aimed to increase patient and clinician awareness about deprescribing unnecessary or risky medications among patients with dementia or mild cognitive impairment. However, the educational deprescribing intervention did not significantly reduce the number of long-term medications and potentially inappropriate medications (PIMs) for older adults with cognitive impairment. Such interventions should focus on older adults taking multiple medications.

Can clinician champions reduce potentially inappropriate medications in people living with dementia? Study protocol for a cluster randomized trial

Michael L. Parchman et al.

Summary: This study protocol outlines a 24-month clinical trial to assess the effectiveness of a clinician champion intervention on de-implementing potentially inappropriate medications in people with dementia. The trial will also examine the impact of the intervention on reducing emergency department visits and hospitalizations attributed to falls, as well as the implementation outcomes of appropriateness, feasibility, fidelity, penetration, and equity.

Summary: This study aimed to assess the association between exposure to potentially inappropriate medications (PIMs) and outcomes including CKD progression, hospitalizations, mortality, or falls in adults with chronic kidney disease. The results indicated that PIM exposure was significantly associated with higher hospitalization rates, mortality, and falls, with the greatest risk observed after more than 1 PIM prescription.

Active deprescribing program in chronic kidney disease patients undergoing haemodialysis

Jyothi Susan George et al.

Summary: The study assessed the medication burden of patients undergoing haemodialysis and found that the implementation of an active deprescribing program significantly reduced the number of medications and pills taken by the patients. The proactive deprescribing program was effective in cutting down polypharmacy in haemodialysis patients, leading to improved quality of life and reduced health hazards associated with medication side effects.
