Whole-Genome Sequencing to Predict Antimicrobial Susceptibility Profiles in Neisseria gonorrhoeae

Summary: By analyzing the genome sequencing and phenotypic antimicrobial susceptibility data of global N. gonorrhoeae isolates, it was found that a single genetic marker, gyrA codon 91, can accurately predict susceptibility to ciprofloxacin. However, multiple genetic markers are needed to accurately predict susceptibility to other antimicrobials.

Efficacy and safety of azithromycin versus placebo to treat lower respiratory tract infections associated with low procalcitonin: a randomised, placebo-controlled, double-blind, non-inferiority trial

Summary: This study aimed to compare the efficacy and safety of azithromycin versus placebo for treating lower respiratory tract infections in patients with low procalcitonin concentrations. The results showed that after 5 days of treatment, azithromycin was not non-inferior to placebo in terms of clinical improvement. However, it is still unclear whether antibiotics are necessary for patients with lower respiratory tract infection and low procalcitonin concentrations.

Providing interpretive comments with therapeutic guidance alongside results reported may assist clinicians who are not well-versed in genetic mechanisms of AMR. However, this is currently not being done in all clinical laboratories. Standardized strategies for AMR gene result reporting are needed.

The widespread introduction and scale-up of genotyping of the gyrA serine 91 genotype in N. gonorrhoeae infections could have significant medical and public health benefits in settings where the majority of gonococcal infections are susceptible to ciprofloxacin.
