Biological variation: recent development and future challenges

Sverre Sandberg et al.

Summary: Biological variation (BV) data has various applications in laboratory medicine, but their usefulness relies on the availability of relevant and reliable BV data. BV data can be obtained through different study designs, including experimental studies and analysis of routine results from laboratory databases. The applications of BV data include setting performance specifications, calculating reference change values, defining individuality index, and establishing personalized reference intervals. This review discusses the major achievements in BV research from the past decade, such as new models and approaches for deriving BV data, the high-quality data delivered by the European Biological Variation Study (EuBIVAS), the Biological Variation Data Critical Appraisal Checklist (BIVAC) and other standards for deriving and reporting BV data, the EFLM Biological Variation Database, and new applications of BV data like personalized reference intervals and measurement uncertainty.

Personalized and Population-Based Reference Intervals for 48 Common Clinical Chemistry and Hematology Measurands: A Comparative Study

Abdurrahman Coskun et al.

Summary: Personalized reference intervals (prRIs) have the potential to improve individual patient follow-up compared to population-based reference intervals (popRI). This study estimated popRI and prRIs for 48 clinical chemistry and hematology measurands using samples from the same reference individuals and explored the effect of using group-based and individually based biological variation (BV) estimates to derive prRIs.

Within- and between-subject biological variation data for serum zinc, copper and selenium obtained from 68 apparently healthy Turkish subjects

Abdurrahman Coskun et al.

Summary: The study aimed to provide updated within- and between-subject biological variation estimates for zinc, copper, and selenium. Significant differences were observed in mean concentrations between males and females, while within-subject biological variation estimates were similar for both genders.

Personalized reference intervals - statistical approaches and considerations

Abdurrahman Coskun et al.

Summary: This study outlines statistical approaches and considerations for establishing and implementing personalized reference intervals (prRI) in clinical practice. Two different statistical models can be used to calculate prRI limits, based on within-person or within-subject biological variation estimates. prRI offers physicians the opportunity to improve interpretation of individuals' test results, though further studies are needed to demonstrate its impact on clinical outcomes.

Personalized reference intervals: from theory to practice

Abdurrahman Coskun et al.

Summary: Using laboratory test results for diagnosis and monitoring requires a reliable reference to which the results can be compared. Currently, most reference data is derived from the population, but this has limitations when used as a reference for individuals. Personalized reference intervals (prRI) based on an individual's own previous test results and estimates of variation can provide a more accurate comparison. With the development of information technology, personalized laboratory medicine has become achievable, and prRI for each individual should be made available as part of the laboratory information system.

Roles of Sex Hormones and Gender in the Gut Microbiota

Kichul Yoon et al.

Summary: The distribution of gut microbiota varies by age and sex, impacting gastrointestinal diseases and being linked to sex hormone metabolism. The concept of "microgenderome" has been proposed, indicating the role of sex hormones on gut microbiota. Further research is needed to universally accept this concept.

Geographic differences in gut microbiota composition impact susceptibility to enteric infection

Ana Maria Porras et al.

Summary: Large-scale studies of human gut microbiomes have shown broad differences in composition across geographically distinct populations, which may lead to varying levels of resistance to specific diseases. Experimenting with microbiome samples from donors representing different countries, researchers observed geographic differences in susceptibility of mice to specific bacterial infections. Further findings suggest that cohabitation of susceptible and resistant mice can enhance immunity to bacterial infections.

The impact of the gut microbiota on the reproductive and metabolic endocrine system

Xinyu Qi et al.

Summary: The gut microbiota, considered a full-fledged endocrine organ, plays a major role in women's reproductive endocrine system by interacting with various hormones. Imbalance in microbiota composition can lead to diseases like pregnancy complications and PCOS, highlighting the need for further research and novel therapeutic strategies.

Personalized Reference Intervals in Laboratory Medicine: A New Model Based on Within-Subject Biological Variation

Abdurrahman Coskun et al.

Summary: The study proposed a simple model to generate personalized reference intervals based on historical results, analytical and biological variation data. Increasing the number of measurements had little impact on the total variation around the true set point, and using >= 3 previous measurement results delivered robust personalized reference intervals.
