Venous Tone and Stressed Blood Volume in Heart Failure JACC Review Topic of the Week

Marat Fudim et al.

Summary: This article discusses the pathological processes contributing to increased cardiac filling pressures in heart failure (HF), with a focus on the venous system and the concepts of stressed blood volume and unstressed blood volume. It reviews how regulation of venous tone affects blood distribution, both between functional compartments and between the pulmonary and systemic circulations, and how these factors relate to hemodynamic abnormalities observed in HF. The article also examines recent methods for estimating stressed blood volume and their application in clinical studies to gain new insights into resting and exercise hemodynamics and therapeutics for HF.

The SGLT2 inhibitor canagliflozin in heart failure: the CHIEF-HF remote, patient-centered randomized trial

John A. Spertus et al.

Summary: Large traditional clinical trials have shown that sodium-glucose co-transporter 2 inhibitors can improve symptoms in heart failure patients. In a new trial conducted remotely (CHIEF-HF trial), canagliflozin was found to significantly improve symptoms in heart failure patients, regardless of ejection fraction or diabetes status.

Device-Based Solutions to Improve Cardiac Physiology and Hemodynamics in Heart Failure With Preserved Ejection Fraction

Summary: Heart failure with preserved ejection fraction (HFpEF), characterized by rapidly increasing prevalence, elevated mortality and rehospitalization rates, has led to the development of device-based solutions to target its associated symptoms. Although promising results have been shown in studies, no device-based therapy has been approved for HFpEF treatment so far.

Summary: This study evaluated the frequency and prognostic implications of urgent heart failure visits in patients with HFpEF. Urgent ambulatory HF visits requiring intravenous diuretic treatment were found to have similar prognostic importance as HF hospitalizations in HFpEF patients. The inclusion of urgent HF visits in a prespecified composite endpoint reinforced the treatment efficacy of sacubitril/valsartan compared with valsartan in the clinical trial.

Beta-blockers and inhibitors of the renin-angiotensin aldosterone system for chronic heart failure with preserved ejection fraction

Summary: Evidence suggests that MRA and ARNI treatment in HFpEF may reduce heart failure hospitalization but have little impact on cardiovascular mortality and quality of life. BB treatment may reduce the risk of cardiovascular mortality, but further trials are necessary. Current evidence for BBs, ACEIs, and ARBs in HFpEF is limited, and their use is not supported in the absence of alternative indications. MRAs and ARNIs are likely effective in reducing heart failure hospitalization, but the treatment effects are modest, highlighting the need for improved patient stratification and new therapeutic approaches.
