ABM Clinical Protocol #37: Physiological Infant Care-Managing Nighttime Breastfeeding in Young Infants

Deena Zimmerman et al.

Summary: The ABM aims to develop clinical protocols for managing common medical problems that may affect breastfeeding success. These protocols are guidelines and not exclusive treatment plans or medical standards. Variations in treatment may be necessary according to individual patient needs. The ABM empowers health professionals to provide safe, inclusive, patient-centered, and evidence-based care. The use of gender-inclusive language may be transitional or inconsistent across protocols due to linguistic and cultural nuances and insufficient research on the experiences of gender-diverse individuals.

Determinants of exclusive direct breastfeeding using constructs from the breastfeeding relationship scale

Natsuko K. Wood et al.

Summary: This study aimed to examine the relationship between constructs of the Breastfeeding Relationship Scale and exclusive direct breastfeeding (EDBF) in US breastfeeding dyads in the first 3 months. The results showed that Mother-Infant Breastfeeding Interaction and Breastfeeding Synchronicity were related, and this relationship was mediated by Perceived Adequate Milk Supply. Perceived Adequate Milk Supply and prior breastfeeding experience were found to be major determinants of EDBF.

Summary: Through the lens of critical race theory and the social-ecological model, recommendations to address breastfeeding disparities and inequities among US Black women include increased communication and educational strategies by healthcare professionals, recognizing implicit bias and systemic racism, early prenatal and ongoing postpartum breastfeeding support, enhanced community support, and the development of breastfeeding groups by and for Black women. Additionally, policies such as paid maternity leave and workplace support for milk expression are essential for institutional support.
