Summary: The optimal consolidation strategy for newly diagnosed primary central nervous system lymphoma (PCNSL) remains controversial between autologous hematopoietic cell transplant (auto-HCT) and whole-brain radiotherapy (WBRT). A systematic review and meta-analysis of randomized controlled trials was conducted, revealing no significant differences in survival outcomes between auto-HCT and WBRT consolidation, but a higher degree of neurocognitive decline associated with WBRT. Individualized decision-making based on age, frailty, and co-morbidities is recommended.

PCNSL occurs primarily in older patients and has a better prognosis, while SCNSL can occur concomitantly with systemic lymphoma or later at relapse and generally has a worse prognosis. The treatment strategies for both types are also different.

Long-term efficacy, safety and neurotolerability of MATRix regimen followed by autologous transplant in primary CNS lymphoma: 7-year results of the IELSG32 randomized trial

Summary: This study reported the results of a trial involving 219 HIV-negative adults with primary CNS lymphoma (PCNSL). The results showed that MATRix significantly improves outcome in patients with PCNSL, with a long-term overall survival rate of 21%-56%. Addition of rituximab benefits patients, and the efficacy of whole-brain irradiation and autologous transplantation is comparable. Salvage therapy was ineffective. Importantly, MATRix and ASCT did not increase non-relapse mortality or second tumor incidence, and the side effects differed between WBRT and ASCT.
