Sepsis-associated acute kidney injury-treatment standard

Alexander Zarbock et al.

Summary: Sepsis is a harmful response to infection that can cause life-threatening organ dysfunction, with sepsis-associated acute kidney injury (SA-AKI) being the most common form of organ dysfunction. Sepsis contributes to a majority of acute kidney injuries in critically ill adults. While advancements have been made in understanding the risk factors, pathobiology, treatment response, and renal recovery of SA-AKI, it still remains a critical clinical condition and a major health burden. Further studies are needed to improve the outcomes of SA-AKI in both the short and long-term. This review discusses current treatment standards and novel developments in the pathophysiology, diagnosis, outcome prediction, and management of SA-AKI.

Cytokine hemoadsorption with CytoSorb® in post-cardiac arrest syndrome, a pilot randomized controlled trial

Celine Monard et al.

Summary: This study investigated the feasibility, safety, and efficacy of hemoadsorption (HA) with CytoSorb in cardiac arrest (CA) survivors at risk of post-cardiac arrest syndrome (PCAS). The results showed that HA was feasible and safe in CA survivors and was associated with a reduction in cytokine levels. Further clinical trials are needed to determine the role and value of HA after CA.

Replenishing HDL with synthetic HDL has multiple protective effects against sepsis in mice

Ling Guo et al.

Summary: Sepsis is a major health issue with high mortality and limited treatment options. The abundance of high-density lipoprotein cholesterol (HDL-C) is reduced in septic patients, and even lower in non-surviving patients. By supplementing synthetic HDL (sHDL), the survival rate of septic mice was improved along with kidney function and reduced inflammation. sHDL treatment was found to inhibit inflammatory pathways and protect mice from sepsis.

First Recombinant High-Density Lipoprotein Particles Administration in a Severe ICU COVID-19 Patient, a Multi-Omics Exploratory Investigation

Sebastien Tanaka et al.

Summary: This study investigated the use of recombinant HDL particles in a severe COVID-19 patient. The results showed that recombinant HDL increased ApoA1 abundance, increased the proportion of large HDL particles, and decreased the abundance of inflammatory proteins. Levels of inflammatory markers and cytokines also decreased during recombinant HDL infusion. Therefore, recombinant HDL infusion may be a potential therapeutic strategy for COVID-19 patients.

Relationship between liver dysfunction, lipoprotein concentration and mortality during sepsis

Sebastien Tanaka et al.

Summary: The study found that low lipoprotein concentrations were associated with mortality in septic patients, while sepsis-associated liver dysfunction was linked to higher mortality rates. In patients with liver dysfunction, there was a lower HDL-C concentration, but it did not impact mortality rates.

Summary: Viral infections can cause cytokine storms and multiorgan failure in individuals with immunosuppression or specific genetic background. This study examined the use of an ApoA-I-containing HDL mimetic in four severe COVID-19 patients with cytokine storms. The treatment resulted in early clinical improvement in three patients and provides initial safety and proof-of-concept data for further evaluation.

High levels of gut-homing immunoglobulin A+ B lymphocytes support the pathogenic role of intestinal mucosal hyperresponsiveness in immunoglobulin A nephropathy patients

Summary: Research showed that IgAN patients have connections between the intestine and kidney, with elevated levels of microbiota metabolites and B-cell activating factor (BAFF), leading to disruption of intestinal IgA class switch. There are significant differences between IgAN patients and those with non-IgA glomerulonephritides, providing important insights for novel targeted therapies.

Summary: The study found that low levels of plasma apolipoprotein A1 (ApoA1) and apolipoprotein B (ApoB) in COVID-19 patients are associated with increased risk of developing severe disease, while the ApoB/ApoA1 ratio does not show a significant association with severity. Additionally, low ApoA1 levels are linked to higher all-cause mortality rates.

Summary: HDL not only participates in cholesterol transport, but also exhibits anti-inflammatory, anti-apoptotic, anti-thrombotic, and anti-infection properties. Changes in the amount and composition of HDL during sepsis lead to dysregulated host immune response and endothelial dysfunction. The multifaceted relationship between kidney dysfunction, dyslipidemia, and inflammation encourages further exploration of connecting mechanisms.

Lipid and lipoprotein predictors of functional outcomes and long-term mortality after surgical sepsis

Faheem W. Guirgis et al.

Summary: In this study of surgical sepsis patients, lipoproteins were not found to predict poor performance status at 1 year. ApoA-I levels, Charlson comorbidity scores, and duration of vasopressor use were identified as predictors of 1 year survival. These findings suggest a role for cholesterol and lipoproteins in the pathobiology of sepsis.
