Transition Preparation for Young Adolescents with Congenital Heart Disease: A Clinical Trial

Quality Indicators for Youth Transitioning to Adult Care: A Systematic Review

This study identified quality indicators for transition to adult care that are broadly applicable across different chronic illnesses and health systems. The findings suggest that while most quality indicators for transition are patient-centered, very few were developed with input from youth and parents/caregivers, and none focused on equity.
