Quality Indicators for Youth Transitioning to Adult Care: A Systematic Review

Summary: Transition from pediatric to adult care is associated with adverse health outcomes for adolescents with chronic illness. This study identified quality indicators for transition to adult care that are broadly applicable across different chronic illnesses and health systems. The findings suggest that while most quality indicators for transition are patient-centered, very few were developed with input from youth and parents/caregivers, and none focused on equity.

Transfer and transition practices in 96 European adult congenital heart disease centres

Summary: The study surveyed existing transfer and transition programmes in European ACHD centres and found that nearly half of the centres offered a formal transition programme, while almost all centres had structured transfer from paediatric to ACHD care. However, most programs lacked an age and development-based approach despite some improvements since 2009.

Mechanisms of impact and experiences of a person-centred transition programme for adolescents with CHD: the Stepstones project

This consensus document addresses issues and practices for transition and transfer of adolescents with CHD across different income countries.
