Summary: This study found that upfront cytoreductive nephrectomy (CN) is associated with improved clinical outcomes and overall survival in patients with metastatic renal cell carcinoma (mRCC) treated with immune checkpoint inhibitors or targeted therapy. This suggests that upfront CN still has a significant role for selected patients in the era of immune checkpoint inhibitors.

Summary: The study indicates that previous nephrectomy has a positive impact on the outcome of pretreated mRCC patients receiving immunotherapy, especially for those with Meet-URO scores of 1 to 3. However, this benefit is not evident for IMDC poor-risk patients (including the Meet-URO score groups 4 and 5) and some IMDC intermediate-risk patients.

Summary: The study demonstrated that the combination of lenvatinib with pembrolizumab led to significantly longer progression-free survival and overall survival than sunitinib, while the combination of lenvatinib with everolimus showed longer progression-free survival but not overall survival compared to sunitinib. Adverse events were more frequent in patients receiving lenvatinib-based treatments.
