Vaginal morphology and position associated with prolapse recurrence after vaginal surgery: A secondary analysis of the DEMAND study

Comparison of the Axes and Positions of the Uterus and Vagina Between Women With and Without Pelvic Floor Organ Prolapse

Summary: This study aimed to analyze the role of the axial positions of the uterus and vagina in providing pelvic floor support and to explore the pathogenesis of pelvic organ prolapse. Magnetic resonance images were used to measure and compare the lengths and angles of the uterine body, cervix, and vagina in women with and without prolapse. The results showed that the prolapse group had smaller angles between the uterine body and cervix, cervix and upper vagina, uterine body and pelvic inclination correction system (PICS) line, cervix and PICS line, and lower vagina and PICS line. In addition, the prolapse group had longer cervix length and shorter middle and lower vaginal lengths. The coordinate system analysis revealed that the uterus and vaginal axes were shifted backward and downward in the prolapse group.

Defining mechanisms of recurrence following apical prolapse repair based on imaging criteria

Summary: This study aimed to identify mechanisms and contributors of anatomic recurrence after vaginal hysterectomy with uterosacral ligament suspension (native tissue repair) vs transvaginal mesh hysteropexy surgery for uterovaginal prolapse. The primary mechanism of prolapse recurrence was apical descent, with greater inferior descent of the vaginal apex and perineal body, lengthening of the anterior vaginal wall, and increased size of the vaginal introitus associated with anatomic failure. Further studies are needed to provide additional insight into the mechanism by which these factors contribute to anatomic failure.

Summary: A strategy has been successfully developed to quantify structural-specific preoperative impairments and improvements after prolapse surgery. Early findings suggest that prolapse surgery is more successful in restoring normal anatomy at Level I than Level III. Improvement in paravaginal location is significantly correlated with apical elevation.
