Recombinant human thrombopoietin (rhTPO) of different dosing regimens for refractory/relapsed primary immune thrombocytopenia: a multicenter, randomized controlled trial and pharmacokinetics study

Xiaofan Liu et al.

Summary: This study aimed to find an optimal dosing regimen of rhTPO for ITP patients who had failed or relapsed after glucocorticoid therapy. Once daily 15000 U rhTPO for 14 injections yielded the greatest median increase in platelet count (167.5 x 10(9)/L) and the highest total response rate (63.2%). Once every other day 30000 U rhTPO for 7 injections rapidly increased platelet counts in the first 7 days. All 4 regimens were safe and well-tolerated.

Discovery of a novel megakaryopoiesis enhancer, ingenol, promoting thrombopoiesis through PI3K-Akt signaling independent of thrombopoietin

Long Wang et al.

Summary: In this study, a drug screening model was established using machine learning to predict the potential active compound ingenol for the treatment of radiation- and chemotherapy-induced thrombocytopenia. In vitro experiments and animal models were used to demonstrate that ingenol promoted megakaryocyte differentiation and platelet production. The activation of the PI3K/Akt signaling pathway was found to be involved in ingenol-induced megakaryocyte differentiation.

Shiwei Wang et al.

Summary: The active ingredient wogonin from Herb Paris was found to inhibit the quorum sensing system of P. aeruginosa PAO1, reducing virulence factor production and biofilm formation, and attenuating pathogenicity. This suggests wogonin as a potential candidate drug for combating P. aeruginosa infections by interfering with quorum sensing.

Computational modeling of stem and progenitor cell kinetics identifies plausible hematopoietic lineage hierarchies

Lisa Bast et al.

Summary: Traditionally, hematopoietic stem cell differentiation was believed to occur in a specific hierarchical manner, but recent research has shown alternative pathways. Through experimental data, mathematical models, and samples from healthy human donors, it was found that the classical hematopoiesis model is preferred in most cases, with exceptions where differentiation of multipotent lymphoid progenitors into granulocyte-monocyte progenitors is plausible.

Summary: This study aimed to evaluate and compare IgG binding to megakaryocytes in bone marrow of ITP and MDS patients. High levels of IgG binding were found in both ITP and MDS patients, suggesting a potential immune-mediated mechanism underlying platelet underproduction in these disorders.

Airborne particulate matters induce thrombopoiesis from megakaryocytes through regulating mitochondrial oxidative phosphorylation

Xiaoting Jin et al.

Summary: The study found that exposure to PM promotes thrombopoiesis in a concentration-dependent manner, inducing maturation and development of megakaryocytes by causing morphological changes, DNA ploidy occurrence, and alterations in biomarkers for platelet formation. The proteomics assay revealed that the main metabolic pathway regulating these changes was mitochondrial oxidative phosphorylation (OXPHOS). The findings suggest a potential perturbation of haze exposure in thrombopoiesis from megakaryocytes, providing new insights for assessing health risks from PM pollution.

Summary: ITP is a heterogeneous acquired disorder characterized by isolated thrombocytopenia, with its exact pathogenesis still unclear and diagnosis made by exclusion. Further studies are needed to understand the pathophysiology of ITP and explore alternative medications due to adverse effects of first-line drugs.

Charlotte A. Bradbury et al.

Summary: In a multicenter, open-label, randomized trial on patients with immune thrombocytopenia, 91.5% of patients who received mycophenolate mofetil plus glucocorticoids had platelet levels above 100,000 per microliter, compared to 63.9% of patients who received glucocorticoids only. Quality-of-life measures of physical function and fatigue were worse in the mycophenolate mofetil group.

The thrombopoietin receptor: revisiting the master regulator of platelet production

Ian S. Hitchcock et al.

Summary: Thrombopoietin (TPO) and its receptor MPL play essential roles in platelet production and hematopoietic stem cell maintenance, with dysregulation contributing to hematological disorders; abnormalities in the TPO-MPL axis can lead to bone marrow failure or pathological myeloproliferation; recent resolution of the debate over how TPO binding activates MPL holds potential for better-targeted therapies in blood disorders.

Summary: Activating mutations in the BRAF-MAPK pathway have been reported in histiocytoses, hematological neoplasms with widespread pro-inflammatory myeloid cells. In this study, an activating mutant BRAF in hematopoietic stem and progenitor cells was shown to induce oncogene-induced senescence response, leading to myeloid restricted hematopoiesis, inflammation and histiocytosis.
