Summary: The burden of cirrhosis from nonalcoholic fatty liver disease (NAFLD) is becoming widespread in the United States. There needs to be greater awareness among endocrinologists, who often see but may miss the diagnosis in adults with obesity or type 2 diabetes mellitus (T2D) who are at high risk. Recent studies suggest that glucagon-like peptide-1 receptor agonists (GLP-1RAs) have beneficial effects on nonalcoholic steatohepatitis (NASH) in this population.

Role of the ghrelin system in alcohol use disorder and alcohol-associated liver disease: A narrative review

Summary: Unhealthy alcohol consumption is a global health issue that has adverse consequences on individuals, public health, and society. Alcohol use disorder (AUD) and alcohol-associated liver disease (ALD) are the most common pathologies among alcohol-related diseases. The liver is particularly vulnerable to alcohol-induced damage due to its role in alcohol metabolism. Currently, the only effective approaches to treating ALD are alcohol abstinence or liver transplantation. Ghrelin, a hormone primarily produced by the stomach, has dual effects on addictive behaviors and metabolic pathways, making it a potential therapeutic target for AUD and ALD. This review discusses recent advancements in understanding the peripheral and central functions of the ghrelin system and its involvement in AUD and ALD pathogenesis. The correlation between blood ghrelin levels and alcohol use or abstinence is examined, as well as the role of ghrelin in alcohol-seeking behaviors and the development of fatty liver. A better understanding of the ghrelin system could lead to improved treatments for AUD and its medical consequences, including ALD.
